Incomplete Bowel Movements

Stool remains in my rectum every time I have a bowel movement. The stool varies from hard to soft, depending on my diet that week. No diet change makes 100% of the stool pass. No matter how long I sit on the toilet, I cannot make the final portion of stool pass. The problem has been there for at least several years, making me believe that there is some sort of nerve damage or anatomical malformation in my rectum. What can I do if changing my diet seems not to work (I have tried more/less fiber, more/less water, and more/less salts)? What may the problem be, and will it be possible to fix, or will I have to continue the rest of my life like this?

Do you also have a problem with constipation?  Constipation does not mean hard stools necessairly but it can also mean not being able to get enough stool out.  IBS is a real diagnosis, even though it is a diagnosis of exclusion (fibromyalgia is also a diagnosis of exclusion but I assume you would consider that a real diagnosis and it has many treatments).  It is considered to be a motility disorder.  There are causes of IBS and there are many treatments (including things like prescription medications) to help with IBS.  You might also have pelvic floor dysfunction which I would recommend researching.

From what I understand though, IBS isn't a real diagnosis, and is just a term given to any condition which has certain symptoms. I would like to actually determine what the cause of the problem is, and try to fix it. Any thoughts?

Welcome to the gastroenterology community!  IBS or irritable bowel syndrome has a common  symptom of "feeling of incomplete evaluation".  Have you seen a gastroenterologist about this?  I would recommend undergoing some testing to determine if this is the problem or if there is a different problem.
