Infant not eating well

I am a mother in need of some advice for my 14 month old daughter, can a doctor please answer my questions and possibly give me some ideas to work with? My daughter has been having eating issues since she was born.  When she was first born she had an intolerance to the formula the pediatrician put her on.  She constantly threw up large portions of her formula after every feeding, whether it be right after or 3 hours after each feeding. We tried several things Enfamil Lipil, Enfamil Gentlease, and then ended up on Enfamil Prosobee.  That seemed to help for the most part, however when she got sick with a gastrointestinal bug at 7 months old and 9 months old for weeks at a time, at which point she ceased to eat her baby food and would only take formula.  

We've tried every kind of baby food known to man (I think) and she won't even let you near her mouth with it.  Also she refuses to let you come near her mouth with a spoon.  Once we moved on to trying to eat more solid foods for a while she would only eat crunchy foods, mostly crackers and other times no solid/baby food at all...just formula.  She suffers from severe constipation that we have tried to remedy with Metamucil, Mineral Oil, Prune Juice, etc. and none of it has worked well at all.  Still constipated, just recently her bowel movements turned to a rubbery consistancy and wouldn't come out without some help.  I'm sure the constipation is from not eating solid foods but I can not get her to eat for the life of me.  I've tried everything. She won't touch fruits, vegetables, and now is going back to refusing everything you put in front of her.  We've been to a speech therapist and they say she has a texture issue, but how long until we try and do something about it. She can't be on formula forever.  She was never tested for Reflux when she was little and they say all the throwing up may have caused a texture adversion.  Long story shortened.....I'm at my whits end crying all the time because my beautiful baby girl will eat nothing but formula, and I'm truely and deeply concerned for her health.  At 14 months she should want to eat everything in sight!! Do you think she may have a gastrointestinal issue that would require and Upper GI through Radiology or what do you think might be wrong with my baby?  I've been wondering if a gastric outlet problem might be to blame but I didn't know if they could tolerate liquids and not solids with something like that. I work in Radiology in a hospital and I'm clueless as to what might be happening to my little girl. Somebody please try and help me I'm begging you....Lord knows her doctor isn't doing a very good job of it.
