Jaundice confusion

This query is about one of my nephews,28years old who had a fever for 3-4 days for which he took Paracetamol 500 tabs as when required but not in excess.Then he noticed that his urine was yellow in the morning which cleared up later in the day after taking enough water.His eyes were very light lemon but his stool was whitish.He got his blood tested and his Serum Bilirubin was 6.6 and Direct Bilirubin as 5.6mg/dl.On consulting a doctor the next day  found him clinically to be mildly jaundiced and ordered blood tests for jaundice and ultrasonogram of upper abdomen.
     His ultasonogram was reported as mild enlargement of liver and thickened enlarged gall bladder with plenty of sludge in it  but CBD was normal in size.No stone or calculus was seen in gall Bladder and CBD.
His blood test report is confusing-his total bilirubin was 6.6 previous day was now 8.11(of course from a different lab).Direct bilirubin 4.92 mg/dl.His liver enzymes were very high with SGOT 2469,SGOT 1196 GGT 110 but S.Alk.Phosphatase was at normal of 62 U/L..
          My query to you is as to what does my nephew really have-obstructive jaundice (high direct bilirubin with enlarged gall bladder wfilled with sludge) or hepatocellular jaundice with such high SGOT and SGPT?
The doctor is confused so I am asking you experts.
