Liver or Gall Bladder? Maybe Colon issues?

I am 35, female, 6'1" and 207lbs.  Over the past 5 months I have lost just over 30 pounds.  I have been tired and nauseous almost daily regardless of the amount of sleep I get.  I have had an MRI that showed multiple swollen lymph nodes in my neck, and the base of my tongue is thick, but with no masses.  I also have three thyroid nodules, two on the left, and a larger one on the right (8mm),  My thyroid levels are all normal.  I have also been having sharp, occasional pain on the right side of my chest, just about where the bra line hits, other times it is dull and nagging.  I am also feeling pain under my right shoulder blade.  My blood work over the last month shows my platelets dropping.  On Dec 3rd, it was 269, Dec 19th, it was 239 and on Dec 21st it was 218.  I have not been checked since then.  My RBC is borderline low at 3.76.  I am also having problems with constipation and occasional diarrhea.   An ultrasound did not show gallstones and the er doctor didn't say anything about the liver, spleen, kidney, or gall bladder ultrasounds when I was evaluated last month.  My blood work did show that my d-dimer was high at 735. I also have a total bilirubin level of 2.2 (the range is .1-1.2).  My MCH is slightly elevated at 34.2.  All other blood values have been normal.  Does anyone have any ideas what might be wrong?  I was told in 2003 that I had a fatty liver, but nothing was done or followed up.  Does anyone have any ideas?  I am worried about the platelets dropping, coupled with being very tired all of the time and really sick to my stomach.  I also feel like a I have a rock in my stomach half the time.
