My fiance is complaining of abdominal pain??

My fiance is 27yr old male.Yesterday when he was on his way to work he had to get off the bus and throw up, and that he felt like he was going to pass out and a while back he had a vasovagal reaction while in the mall.he was complaining of abdominal cramping,nausea,headache,and being light headed.He just started feeling like this after he smoked a cigarette.And he still feels like this, when he told me last night that he felt like that i assumed maybe his sugar was low or something. He has taken pepto bismol to relieve the pain but it hasn't stopped. What should I do? what could it be?
