Where was the stone found?  I can't quite tell from your post if it was in your gallbladder or elsewhere.  A .4 mm stone is pretty small, but it may be blocking the flow of bile from your gallbladder.  Are you still jaundiced or has that cleared up?

Here's a quick anatomy lesson that may help you understand what's happening.  The liver is in the upper right area of your abdomen and produces bile which helps digest fats.  The bile flows to the gallbladder where it is stored.  When you eat a high-fat meal, the gallbladder contracts and sends the bile down the common bile duct to the upper part of the small intestine.  Between the gallbladder and small intestine, the pancreatic duct joins with the common bile duct.  Digestive enzymes from the pancreas mix with bile from the liver in the common bile duct and go to the small intestine.  The small intestine is where most digestion and nutrient absorbtion occurs.

Your problems could be anywhere from the liver, to the gallbladder, to the pancreas or somewhere in the bile ducts.  If your liver tests are normal, it's time to look somewhere else along that entire system for the problem.  For example, even a tiny stone such as that found on your ultrasound may have become lodged somewhere in the common bile duct.  If the flow of bile or pancreatic enzymes is slowed or stopped, that can cause the symptoms you describe.

Generally a gallbladder problem will cause pain in the upper right side of the abdomen.  A pancreas problem will cause pain on the upper left side.  A small stone or sludge in the common bile duct can upset both the liver and the pancreas.  This can be treated with a procedure called ERCP - kind of a super-endoscopy where the doctor can flush out the duct.

Since you're still so miserable and the medications aren't helping, it's time to go back to your gastroenterologist.  In the meantime, try to determine what foods make your symptoms worse.  Something tells me you probably need to avoid fatty foods and alcohol right now.  Also make a note of anything that makes your symptoms better.  This information can help your doctor with diagnosis and treatment.  Best of luck to you, and don't give up!
