Pinch Rib Sternum

It seems like on and off for several years I have had a burning pinching sensation under my rib sternum intersection upper right abdomenal (abdominal) area.  I had my gallbladder removed 3 years ago, no stones, one attack, hida showed it was not working at all.  Lately, it seems that the pinching is more constant.  When I drink cold liquids, it seems I can feel it in that area as well as below the sternum.  I have a moderate reflux (barium test), take Prilosec, absolutely works great for the reflux.  When I exercise (run) really hard, it feels like I was punched in the stomach. But not badly enough to stop the exercise.  The pinch is not incapacitating, doesn't radiate, no nausea, stools are usually soft to runny.  Also had complete stool work up 3 years ago because of the runny nature , nothing unusual, no constipation and I have regular (daily) bowel movement.  I drink 10-16 oz white wine with dinner which is usually a salad, don't eat spicy foods, can't correlate this pain/pinch with anything.  External pressure doesn't increase it or decrease it.  Had a physical therapist feel under the entire right rib cage for any obvious lumps or swelling, but nothing was alarming.  I can press quite far under my ribcage without pain.  No sensation of bloating.  I have researched possible reasons, know that testing is probably the only real answer, but I have no health insurance.  Is there one area of concern more likely than another?

Various causes of upper GI disease can be considered.  GERD, inflammation of the upper digestive tract, or an ulcer can all lead to the symptoms you are describing.

An upper endoscopy or blood tests looking for a bacteria known as H Pylori can all be considered for further evaluation.  

If the symptoms are related to exertion, you may want to consider a stress test to rule out cardiac disease.  

These options can be discussed with your personal physician.

I also wanted to add that I feel a hard spot (difficult to detect) between my sternum and the horizontal line of my middle ribs.  Wondered about an epigastric hernia.  When I press very hard on that area, I have the same sensation as when running fast, like a stomach ache.

Have you had your liver checked? I'd have your liver enzymes tested. It's a simple blood test, ask your dotor if he wil run a CBC on you and do a full hepatic panel.
Pain in that area can be associated with liver issues. Liver issues can be very sneaky so I'd check it out to make sure you aren't dealing with a liver issue, particuarly with drinking wine daily.
