Female sexual dysfunction with abdominal pain that has been going on for about 2 - 3 months. The pain will range in intensity from simple discomfort to severe and once finally to what I label unbearable, which finally sent me to the ER. Pain originally was only in the right upper side, but now also includes the area above the navel. The pain is deep, sharp and when moderate level or above is also in my mid-back. So far tests have been negative, including blood work, CT Scan,

Esophagus and stomach anatomy. Diverticulosis was found but was not inflamed. I occassional run a low grade temp (99.5-110.5). I have not been able to determine any connection with eating patterns, etc. Dr. has tried

Prilosec otc, & paxil. I have now been given librax to use when the pain in severe, but again I do not experience relief from this either when the pain is at its worst. I get nausious after the pain gets bad enough which often leads to vomiting. I don't have any other gastro systems. Only other guess other than some gastro problem is adhesions from my gall bladder removal in Feb 97. I can tolerate a fair amount of pain, but I'm beginning to wear out! The only other symptons which may or may not be related is my blood sugar levels occassional change. I am NIDDM but am well controlled with diet and exercise only.(5.8 on last A1C) Yet occassionally I have had a high morning reading (e.g.,198) and a couple of times a low reading (60's). This does not happen enough to change the A1C though. Any ideas on the cause or perhaps ideas on living with this pain?

Dear Sandy,
One possible explanation for your symptoms is small intestine disease e.g. Crohn's disease. The pain may be related to pancreatic inflammation although there are several features that make this possibility less likely: a)m absence of risk factors for pancreatitis, b) atypical location of the pain [pancreatitis usually causes pain in the middle of the stomach between the ribs, and c) apparently normal CT scan. Common duct stones could also cause many of the symptoms of which you complain.
Before you accept this pain as being a permanent fact, you should complete the workup to exclude treatable causes. Ask your doctor if he hasdone serum amylase and lipase levels. Discuss the diagnostic benefits of small bowel enema as well as ERCP to check the pancreas and bile ducts.
Best of luck.
