I'm a 32 year old female in fairly good health. I am a bit overweight...15 lbs to be exact. Yesterday morning I had trouble with my bowel movement. I had to strain really really hard. I seriously felt like I was giving birth. The BM was not hard but unusually large and wide. After relieving myself I felt much better. Later on in the afternoon my rectum/anus felt irritated (itching burning sensation) I went to the bathroom and was SHOCKED to see blood on my underwear!!! It was about 2tsp... that's alot! Could I have tore something? I had another bowel movement and let me tell you it was painful.... it felt like glass/fire. It is extremely painful to wipe. When I do wipe there's tiny spots of blood on the toilet paper. The sight of blood worries me. Should I go to the doctor ASAP. I am pretty surre it's coming from my anal area. There doesn't seem to be any blood in my stools... stool color seems to be normal. Please I need your advice.
Oh yeah and another thing.... I had some bloodwork done about a month ago and the ALT (SGPT) liver test was slighty out of range.
normal range is between 6-40 and mine was 45. I also had and elevated ESR of 54. Maybe there is something going on or maybe I am working myself up. I am going to retest sometime this month. Now I am really worried. Need your advice!!

Constipation can indeed to lead to some mucosal tearing and subsequent blood.
However, any blood is abnormal and should be evaluated by a physician. An anoscopy or flexible sigmoidoscopy can be considered to evaluate for hemorrhoids, polyps or other masses.
An elevated ALT should be evaluated with an ultrasound and hepatitis panels. It should be repeated to ensure the levels aren't rising.
An elevated ESR is non-specific and suggests an inflammatory process such as an acute injury or infection.
These options can be discussed with your personal physician.
Followup with your personal physician is essential.
