SEVERE LLQ ABD PAIN W/O BOWEL MOVEMENTS WITH FIST SIZE LUMP?

by Aneliese, Oct 23, 1998 12:00AM

Hi HFC,  I have been having severe LLQ Abd. pain with fist size abdominal mass felt.  I have the feeling that I have an obstruction of some sort.  I have H/O Colitis for past 10 years relapsing /remitting. The pain has been so bad that walking is difficult.  I become sweaty and nausious do to the attacks of pain.  Does this sound like typical flare of colitis?  I really don't want to be scoped again. The only way I am able to have a bowel movement at this time is to push on the area around my rectum and even with doing that I have very small calaber stools and severe abdominal pain. Can hemmorroids get so big that they will block the rectum to cause obstruction?  Any Advice would be appreciated.
Thank you for your kindness, Sincerely Anneliese

Dear Anneliese,
Hemarrhoids would be an unusual cause for the difficulty that you are describing in moving youir bowels.  It is necessary to exclude internal obstructions ( stricture, tumor) rectal pathology (fissures) and functional abnormalities ( anismus, pelvic floor abnormalities, Hirschprung's Disease).  X-rays, endoscopy and anal-rectal manometry are needed to define your problem.

We would be happy to see you in the Division of Gastroenterology of Henry Ford Health System to evaluate your problem.  We can perform all the above tests as well as rectal ultrasound looking for extrinsic causes for your symptoms.  You can make an appoinment with Dr. Fogel, one of our experst in the diagnosis of rectal problems, by calling our Physician Referral Line at (800)653-6568.
