Sharp Incisional(?) Pain Post tp/iat, colectomy, splenectomy surgeries

Yesterday as I was putzing around on my computer I got an extremely sharp pain mid sternum exactly where my incisional scar begins after having had a total pancreatectomy with autologous islet cell transplant (tp/iat), splenectomy (spleen removal), partial (70%) colectomy (partial removal of the colon) and an appendectomy (appendix removal if you didn't already know) thrown in for good measure(!)  It literally took my breath away and brought tears to my eyes it was so sharp.  But it only lasted about 2 mins. max so I blew it off thinking it was just a fluke.  About 5 mins. later it happened again so now it had really gotten my attention.  But, even so, it too went away as quickly as it had started so I once again put it out of my mind.  

Then, later that evening while watching a movie, about 10 mins. into it; it happened again but this time the pain radiated around my ribcage and I felt the urge to "go."  Since I sometimes get a sort of "remnant" of pain I used to get very severely prior to my surgeries, while "going" I thought maybe this, too, would go away when I was done (as does the "remnant" pain I get.)  Well, it didn't go away until I was "forced" into using about 1/3 of the fentanyl lozenges (or lollipops as I call them as they're on a stick) which are prescribed at 1-2 lozenges (400 mcgs. EACH!) for severe pain only.  

Later, the same thing, i.e., using "just enough" to bring it down from severe to bad pain but since I only had the one left and even w/health ins. they're $300 for 12 of them - I had to do that!  Right before the last time, I had eaten 2 small containers of nonfat yogurt which NEVER bothers me - not even when sick w/chronic pancreatitis!  So, now I'm afraid to eat but since I'm still living as a type I diabetic, as I wait for the transplanted cells to function that were taken from the pancreas before removal and transplanted onto the liver , if I don't eat, it wreaks havoc w/my blood glucose levels primarily causing hypoglycemia.  Long story there as some of you may be aware!  

I have felt a sort of 'hard ball" shape directly under the top of the incision recently as well as a smaller one a bit further down.  Could this be scar tissue?  And if so, would it hurt that badly - like a charley horse, tight pain?  Could it be a herrnia?  Same question reg. pain.  Could it be a hematoma?  Again, w/pain?  I got a 'short" burst of it this morning as well and though I plan on getting in touch w/my surgeon (in Minn. - I'm in RI unfortunately but the price to pay if you want the best of the best to do your surgery!)  And possibly my pcp (as my GI/pancreatologist is a 1.5-2 hr drive to get to his office/hospital in Boston) maybe they can at least xray it or use other visual imaging equipment to visualize it to see if it's scar tissue or whatever.  

My surgery was on 9/5/08 so I'm only 8 wks post-op.  Anyway, if anyone else has experienced this (even if a different surgery) could you please reply to at least give me an idea of what it could be and how they'd treat it?  I'm very worried as you can imagine AND because if it continues at that really severe level, I don't have the $300 right now to shovel out on a "lollipop' prescription!  Thanks!
