Should I wait one more year?

by titita, Apr 14, 2005 12:00AM
Dear Doctors:
My name is Raul. I am 52 years old.

Six months ago I had a colonoscopy, and an endoscopy performed, because I got some occult blood in my feces.  The colonoscopy turned out to be OK, (just hemorroids (hemorrhoids)), but in the  Endoscopy  the doctor removed  a 1cm gastric polyp which was biopsied, and he also saw a 1cm Duodenal polypoid Lesion, which was  biopsied as well.(Brunner Gland Hyperplasia).

At that time the doctor advised me he wanted to perform another endoscopy  in 6 months later to see if there have been any changes in my duodenum.

The 6 months went by and I already have my new endoscopy done, which showed the following:

FINDIGS: Smooth submucosal appearing 1 cm approx. lesion in duodenal bulb(biopsied)
ENDOSCOPIC DIAGNOSIS: Smooth submucosal appearing duodenal bulb lesion r/o leiomyoma pancreatic rest/o Brunner gland hyperplasia(biopsied).

The doctor told me that in some occasions biopsies are not very accurate, because they are only done superficially.
So he recommended three things for me to choose:

-Wait one more year and then check it again,
-Do a CT Scan Pancreas, abdominal, and pelvic with contrast
-Perform an Endoscopic Sonogram

Of course, I chose to wait one more year, because I do not see a serious situation. But my wife, who is a very preoccupied person insists that I should not wait one more year, and  I should avoid any risk and should go ahead and see what is it that I have on my duodenum more deeply now.

My question with you doctor is: Can I wait one more year for another check up?, or should I go ahead and do the CT Scans, and the Endoscopic sonogram?

According to your point of view, and considering the experience you should have.  Am I putting my health in risk for waiting one more year, or is my wife correct and I should not delay this any longer. Let me quote that the doctor also suggested that it was better for me to performe the tests, rather to wait one more year.

The lesion that I have in my Duodenum can revert to cancer?
I would like you opinion and you recommendations.

Please answer my question because I really do not know waht to do Thank you very much.

Raul

There are no absolutes in medicine, so I cannot say that waiting one more year would be harmful or not.

It is also difficult to comment on what to do next without being familiar with your case.

If there is concern about the lesion, I probably would lean towards doing further testing.  The CT scan and/or the endoscopic ultrasound are not unreasonable at this point.  At least, if they are negative, you can be rest assured that there is no further spread going on.

However, without knowing your case personally, it is impossible to give a firm recommendation.  You can always arrange for a second opinion in these cases.

Followup with your personal physician is essential.
