Undiagnosed upper abdominal pain

I suffer from severe upper abdominal pain that radiates from just belwo the sternum which i imagine is where the esophageal sphincter is located, the attacks happen mainly at night whether in bed or sat upright. The pain radiates through the back and down the arms the stomach becomes distented and can last for hours.
A barium meal x-ray showed i have a Cascade/Cup and spill stomach, but the doctors and surgeons say that this is not relevant to my condition.
I have suffered with Gerd for 5 years, i do not smoke or drink alcohol, i eat healthy, no sweets, no fatty foods, no large meals, no late night meals, i get regular excercise but i have suffered from tension and stress a lot in the past.
I am taking Lansoprazole(PPI) acid blockers, 30mg in the morning and 30 mg on a night, the problem must be acid related, because 12 hours without acid blockers and the pain start without fail.
I have been for 2 Gastroscopy's, no problems have been found with the esophagus and stomach, the doctors have been very unhelpful and i have never received any type of diagnosisbor even an explanation of what may be causing the pain, so i have been left to scan the internet and try to find a cause and maybe even a cure.

From what i have read so far the closest thing i can find to explain my symptoms is "Esophageal Spasms" and it may be that acid aggrevates the esophagus causing a painful spasm.
But i can have these pains even when i am on the acid blockers, when this occurs i take Buscopan and in order to relax the esophagus, and take Gaviscon to neutralize any excess acids that may have built up but it can still take hours for the pain to go away.

If this is "esophageal spasms" what can i do to stop the attacks or cure the problem? Or could this be something else like a problem with the "Gall Bladder"?

I just wanted to add some additional information, there is almost always a dull ache in the same place where i get the pain, and the ache goes away when i eat, but will come back about an hour later.
The painfull episodes have occured after drinking something too cold or something too hot, but they are not always triggered by heat, they are more often triggered on a night time, but at all times it feels like the pain could start up at any second.
The ache also seems to get temporarily relieved if i arch my back to stretch my stomach, or if i have been doing excercise at the Gym the ache may disapear for hours.

Just to also clarify that i do not take normal Gavison, as this does not neutralize acid, it only provides a protective layer on the esophagus and over the stomach contents, i use gavison "double action" which is also designed to neutralize excess stomach acids.
