Upper abdominal pain - 4 years in 33 year old

by ronnieronnie, Jun 11, 2008 05:21PM
Its been about 4 years since I started with upper right abdominal pain.  Its is not a searing pain but a dull pain that comes and goes.  WHen it comes, its constant for several weeks and then it does away.  Its worse when I sleep on right side.  The pain extends to the back and at times it really becomes throbbing for several seconds and dulls again.  I have been to numerous doctors and was advised I have tumors on my liver, fatty liver and also my gallbladder output was only 13% which the doctor said I should get it removed.  There are no stones or obstructions.  I have diabetes and have not seen any real weight loss due to the condition.  I get diarhea (diarrhea) often and gassy too.  I get nauseaus often and feel like vomiting.  Sometimes if I vomit I feel better.  Overeating is very uncomfortable.  I have a concern removing my gallbladder if it is not kidney stones but am not sure really what to do.  I have lived with the pain for too long and it really concerns me.

13% of gallbladder function is really low. The most common cause are gallstones or biliary sludge - a condensed bile from which gallstones may eventually appear through the time. Ultrasound often miss both conditions, CT or MRI show them better but still not 100%. Even, when there are no gallstones or sludge, "biliary dyskinesia or Sphincter of Oddi dysfunction" (SOD) may be the cause of yor pain. Gastroenterologist should explain you results of HIDA, and what are your options. If the problem is gallbladder emptying, gallbladder removal would be a solution. But if the problem is in the main biliary duct, gallbladder removal wouldn't help. You just need to get exact explanation of HIDA. Other tests of biliary tree are available, if necessary (ERCP, manometry of sphincter of Oddi).

Kidney stones may be seen much more clear with an x-ray. Often, specific substances can be found by urine test. Vomiting which releaves your pain, speaks against kidney stones.

Another (additional) issue may be H. pylori infection of your stomach. Symptoms may be: burping, heartburn, bloating, early satiety. You may want to have a breath test to exclude/confirm H. pylori. Treatment is with antinbiotics.
