Upper right quadrant abominal pain.

I'm 27 years old and have been suffering from upper right quadrant abdominal pain since i was 15yrs old.
There is a knot/lump right under my ribs its been about the same size since the pain started 12 yrs ago. When the knot hurts it also hurt in about the same spot of my back. The knot is about the size of a dime, when it hurts it feels like the muscle is being twisted and burns, similar to cramp and my back hurts in the same spot and feels like it needs to be poped.
The first couple of years that this began the pain would start after being on my feet for an hour or two (i used to waitress), but would subside after resting for a little while. Then the pain would just disappear for a couple of months as if it had never happened, but it would always return in the same fashion. I went to the chiropractor for around six months and felt no relief he said that the pain could be due to the last two vertabra connecting to the pelvis were a little slanted. Visiting the chiropractor never helped me.
After about 4-5 years the pain quit disappearing, it was always there not a constant pain but hurting after being on my feet for extended periods of time, being easily alleviated by resting.
Once again the pain just went away for a about a year, and came back. This time its been getting worse little by little. When i was pregnant with my first child a 23 I could barely do house chores as the pain was so bad. Running errands was torture and going out was nearly immposible. After my son was born the pain continued like before only hurting after being on my feet, but far more frequently and taking longer for the pain to subside. I became pregnant agian at 26 and suffered the same torture as when i was pregnant with my first child. I had a C-secton with both kids. post op was fine with the first no unusual pain with the knot. However with the second C-section the pain was nearly unbearable post-op. The doctor sent me home upon discharge with a Strong pain meds for the pain and they did nothing to help. At this point it doesn't matter if I am sitting or standing the pain is there, the only thing that makes it go away is to lay down.
By the time I went back for my six weeks check up the doctor had already had to call in two refills for the pain meds that were barely helping me sleep throuhg the pain. He had me get some blood work done and made me aa appointment with a surgeon and had my gallbladder removed thinking that would make the pain go away. It now 9 months later and the pain has only got worse. It hurts so bad now that even laying down barely helps, the pain wakes me up at night sometimes.
Once again I've gone to the doctor and they don't say anything about, they act like I'm crazy like I'm making it up. I know what I'm feeling and they seem to not care. I don't want to feel like this forever and I don't want to have to rely on pain medication for the rest of life, its not like it actually helps anyway.
Everytime I go to the Doc. they check to see if I'm sick and there doesn't ever seem to be anything wrong with me other than the pain yet they can't figure out what is wrong with me.
If anybody else has had this problem please let me know if you have had any luck with a diagnosis.

Your pain seems neurogenic to me, realtaed to spinal nerves. Increase in pain during pegnancy could be due to change in posture/ or pelvis. Was MRI done at any time for diagnosis? If not then MRI Spine may shed some light and I think FNAC of nodule will be of some use too. Have you ever tried physiotherapy? If not then try some back exercises and tell me the response.
