What happens when your really bad spleen is not treated?

I have had an enlarged liver and spleen for over 7 years. It has gone untreated as well. I have flair up inwhich i have to rest it for sometimes up to 2 weeks. This time the flair up is really really bad.
A few months ago i was in the hospital and was quickly released (due to not having insurance)... within a few days I could barely get out of bed. I had severe Anemia and diabeties. I had testing done and cared for both myself. My question is.... I know the spleen cant burst with trauma. However; if left untreated will it die and what am i too expect? Will I die too?
The pain is unbearable. I Sometimes i cant tell if Im having another heart attack or if its the spleen. I am taking pancreatic enzymes which help, But, the pain is about the worse I have ever had.
Please don't tell me to see a Dr. or go to the hospital. Dr.'s and hospitals don't want me without insurance. THEY WONT HELP! And don't tell me they will! I have had 5 years of being turned away since loosing my insurance so I know first hand they wont touch me!
I dont mind dying... i just want to know if it will happen or if i will be in continual excruciating pain the rest of whatever life i have left.
