What is this pain?

Two years ago I had a terrible attack..pain in my upper right side radiating to my back. I went to the ER and had many tests. It seemed like gallbladdder issues but the tests were inconclusive.
Last weekend I had a terrible attack..pain that lasted for 5 days. I went on an all liquid diet. I had a nuclear test and blood work. All were normal. My doctor says it may be gallbladder spasms. Could spasms hurt that much? Would having my gallbladder removed help prevent this?
