  45 yr old male. Peptic ulcer as teenager, first treated with vagotomy, later bilroth II.( 25 yrs ago ) In connection with strenous exercise or work get personality change, appear drunk, irritable, cannot concentrate. Wife detects sourly smell from mouth which otherwise is not present. Treated with antibiotics for helicobacter 2 yrs ago, made no difference.
  What can be the problem ??

Dear per,
It is inappropriate to provide a differential diagnosis for your problem.  Patients with gastric surgery can have periods of hypoglycemia ( late dumping) approximately 2-3 hrs after eating.  A low blood sugar can cause confusion, inability to comcentrate etc but would not cause a sour smell.
You should discuss possible explanations for your problem with your doctor.
