black hairy tongue is commonly caused by broad spectrum antibiotics such as clarithromycin or cotrimoxazole. The antibiotics cause overgrowth of certain micro-organisms on the tongue resulting in thickening of the pili of the tongue which gives it a black appearance. Treatment is to increase oral hygiene, brush more regularly or use a hydrogen peroxide & water solution to brush tongue.
