could this be caused by a hiatal hernia

I have Celiac disease for which I am on a strict diet.  I have also been diagnosed with abdominal migraines for which I take injectable Imitrex.  I have always had GERD and had a Mallory Weiss tear in my esophagus several years ago when I was in active labor.   I know I have a hiatal hernia and have had reflux for years.  I also have a chronic pain disorder and a fatal degenerative brain disease.  I recently had a swallowing study done to see if I had aspiration because I had bilateral pneumonia and people with FTD, the brain disease, often have pneumonia.  I did have some aspiration, and the tech. could see the very top of a hiatal hernia but her test didn't go down that far.  But, I didn't have a sig. swallowing disorder.  In the past year, it has gotten much more difficult for me to eat.  I have to eat things that are smooth in consistancy.  If I try to eat something like roast, first it feels like my esophagus is tightening then I immediately take a Valium and Malox.  If it progresses then I will start to have a great deal of reflux, sharp pain in my chest about at the level right underneath my breasts, and I will begin to vomit.  I will continue to vomit for 8-10 hours.  I have been to the ER.  IV antiemetics don't work.  I've tried using my migraine medicine but it doesn't work.  The only thing that works is a large IV dose of narcotics which the ER won't give to me because I am a chronic pain patient on opiates and I am opioid tolerant.  

I was wondering if this could be due to my hiatal hernia?  I have alot of medical problems and my doctor just finished a round of studies for another problem but this is the next thing I'm going to ask him about.

I just wondered if anybody had the same experience.
