I agree with the ultrasound. I would also obtain blood tests to look for hepatitis.
Regarding the chronic back pain, a referral to an orthopedist can be considered. Imaging the back with plain films or an MRI can be considered to determine if there are any bony abnormalities or disc disease.
If the liver enzymes continue to rise despite negative testing, a liver biopsy can be considered.
