There is no definitive studies that I am aware of demonstrating the efficacy of milk thistle.
If there is an association with itching, you may want to consider primary miliary cirrhosis. Obtaining an alkaline phosphatase level can be considered, however a liver biopsy would be needed if this diagnosis is suspected.
This option should be discussed with your personal physician, or in conjunction with a hepatologist.
