gas bubbles w intestineal pain

I have noticed that as of the age 42, if I go over 4-5hrs without eating, when I do eat, it has to be a cup size meal. One that consists of a salad, soup, or small sandwich. If I eat a big meal after waiting this long I will develope gas pockets that hurt severly in my back, on my left side behind my stomach, and in my intestines, right under my breast bone area. The pain is so painful it makes me nauseated, or makes me feel like I have to have a bowl movement. The only thing that  will make me feel better, is if I drink a sprite soda to belch the gas out. Should I be worried? What should I do?
