great pain still, two months after hemerroidectomy and sphcinterotomy

drs., I'm a 43 yr. old male. Have had bad hemerroids all my life- had a hemerroidectomy for internal hemerroids in may, also had an anal fissure. Had a sphcinterotomy first of july. Still in GREAT pain when moving bowels 2 months later. I'm vegetarian, eat a high fiber diet. I'm on: librax,skelaxin,oxycodone,fentanyl patches,and topical metronidazole also. Still the pain is unbearable at times! My colon/rectal dr.is stumped, says my fissure has healed, so he has referred me to a dr. at Rush Presbytarian Hospital in chicago where I am to have a colonoscopy sept. 8th with Dr. Theodore Saclarides.Do you think the procedure will be able to determine the problem I am having?
Do you have any ideas or suggestions as to my problem? Before this, I occasionaly had trouble evacuating my bowel fully, I would have to go back minutes later and "go" again; but not recently. I cannot return to work until this is taken care of as I would lose my job for too much time I've had to take.Please help Dr.-I am at my wits end!

Difficult to say without examination.  An recurrence of the anal fissue can certainly lead to pain, as well as hemorrhoids.  This would likely have been considered by your colorectal surgeon.

Other possibilities can include irritable bowel syndrome or inflammatory bowel disease.  

I agree with the present course, and that a colonoscopy would be the most comprehensive approach to determining the cause of the pain.  

If non-revealing, you can always consider another opinion, possibly a GI specialist.

Followup with your personal physician is essential.

Hi Docs,
Great work you do here.  I have a question regarding someone i know - here are his lab work results on the liver.  I wanted to ask if this is any indication of something serious?  the doctor gave him liver cleansers - milk thistle and giseng something - instead of a liver medication.
The cleasers are working well and he is improving.  These are the tests after one week of cleansing the system.

The doctor had suspected hepatis B and that came up negative.
whats your take with these results.  He doesnot have a stomach problem or pain.  He has jaundice which is clearing after he started taking the cleansers.  The color in his feases is coming back to normal. He has regained his appetite since he started cleansing the leaver
- there tests were taken before the improvements.


Additionally, he took a sonogram which showed a normal stomach but they could not rule out hepatitis.  The spleen was slighly enlarged.

Chicago and the name of the Hospital that you will be going to

for this.  I am in really bad shape, not only physically but

locally (where I live) and am unable to find any decent medical

care for IBD.  I have had Crohn's for over 20 years and need

help.  Please E me so we can talk.  You'd be doing a stranger a

worse way.  I've been very ill for over a year with no real help

from anyone.  I went out of town already to a major (big, real

big)Clinic - if that gives you a hint and was not looked after

appropriately at all.  Very disappointed in receiving the kind

of help that I received from them, so I'm moving on until I do

or collapse trying to. Some of your situations are similar to

mine and almost exactly.  I hope to hear back from you soon~
