hiatal hernia/choking

My 87 year-old mother has had a hiatal hernia for years and also has osteo.  Recently experiencing choking  (I noticed signs weeks ago).  She stated that she is going to choke to death.  Considering her age and osteo I'm not sure what to do as her daughter/caregiver if she stops breathing while I am with her.  DNR is in place and I have medical power of attorney.  I'm unsure about discussing this with her for fear it will only make her more nervous.  Will the choking get worse with time and will she eventually choke to death?
