high liver enzymes, fatty liver, edema, not cancer

My mother has an autoimmune disease called dermatomyositis, she was on methotrexate and just recently discontinued due to high liver enzymes.  She had lumpectomy for breast cancer 5 yrs ago, still on tamoxifen.  She had an increase in liver enzymes, normal in nov '09, march '10, marked increase, Ultrasound showed low density lesions, had CT which said it was riddled with mets, then a biopsy friday showed only fatty liver, all cells were benign.  Meanwhile, her protein and albumin are low, her calcium is low, kidney function is ok, but she is extemely edematous 4+, and even  her arm is getting edema.  The lasix does not produce much diuresis, she is short of breath, but they did not find CHF, she says she feels gurgly, but lungs are clear, she did  have small pleural effusions in lower lobes, but no treatment.  She was in the hospital for 3 days, they did not give her any lasix during that time, just hydration and antibiotics for a UTI.  She is back home on low dose lasix, 20mg. She also has diabetes and is on glimepiride and NPH insulin since no other insulin has worked.  She has a very complicated history, but i appreciate any thoughts on what to look for. thanks.
