nausea and loss of appetite

Hi Doctor
I am a 48 year old female.  For approx. 17 years I have been having bouts which last about 2 weeks with the following symptoms: severe fatigue, no appetite, nausea (no vomiting), weak wobbly feeling legs, occasionally a strange numbing chilling sensation throughout my body and achiness.  The frequency of the bouts vary and at one point they went away for about 6 years.  Since October of 2006 they seem worse and more frequent.  I have had 5 bouts of this illness since October and it is really starting to interfere in my life.  When I have one of these bouts I have a real hard time working and enjoying anything.  I just feel miserable!  Even though I am exhausted I seem to have some insomnia.  In between bouts I feel great.  Every single bout with this that I have ever had started the day before or the day that my period ended.  

27 years ago I had some sores in my pubic area and I went to a doctor. He took a real quick look and said I had herpes.  That is the only time I ever had sores.  Is it possible the herpes virus is causing these symptoms even though I have no sores?  I have been to several doctors who run routine blood tests and find nothing abnormal.  What do you suggest I do?

I would suggest an upper endoscopy.  This test can look for blockages and strictures that may be causing your symptoms.  If herpes is a concern, the endoscopy can determine if there are any suspicious herpes lesions within the esophagus.

I would also consider an abdominal ultrasound to look at the liver and gallbladder.  

More specialized tests can be considered, such as a gastric emptying scan looking for gastroparesis as well as a 24-hr pH study to rule out GERD.

These options can be discussed with your personal physician.
