pains in upperback,right shoulder, right side below breast

I have been having all these symptoms for a few months, dr. sent me for an ultrasound looking for gallstones. None was found, but did show fat in my liver?  Dr. said this would not cause the pain. The pains still persist. I get an attack of pain almost every day, sometimes quite painful which last about 1/2 hour or so. Last 2 days I am now getting pain on left side just below ribcage.  My dr. suggested seeing a G.I. Specialist, but I don't have health insurance and can't afford the visit. I had to borrow money for the ultrasound. ANy idea what could possibly be causing this? Maybe an Ulcer?
