perforation peritonitis with septicaemia with MOD

Hello Doctor

We need your suggestion. My father has went through the major surgery of perforation

peritonitis with septicaemia with MOD in K/C/O COPD  with AF with IHD.
Operation was done on 20-08-2011. Today(01-09-2011) his consious level is increase. However

his SGPT:221 SGOT: 400.
Also
Creatinine is 5.5
Bilrubin is 8.6
Urine o/p per day is averagely 850 ml

He has been dialysed for 6 times since 20-09-2011 till now.

He is admitted in one of the wellknow hospital at aurgabad. dhoot.

Kindly suggest how we can bring the SGPt and SGOT in range.

Hi there!

The SGOT and SGPT are just enzymes/ markers and not very high in the case. It is the underlying disease that needs to be corrected and not just the markers. He has other critical issues to be taken care of at this stage i.e. increased bilirubin, perforation peritonitis, septicemia, renal issues (creatinine 5.5mg/dl) and MODS. He would need a muli-disciplinary/ specialty approach for appropriate treatment (intensivist/ hepatologist, gastroenterologist and nephrologist). Critical care may be required if suggested. It would be best to discuss the situation in detail with his treating doctor.
      Also, this is the orthopedics community, you may try re-posting in the relevant forum.

My father has taken 60 acitrom 1mg tablet regularly and got affected with saviour cough and stomach pain problem. he was admitted in ICU due to blood got thinner and doctor have given him 4 bags of FFP (plasma). He felt well after 2 days. However on 4th day he got very saviour pain in stomach and got to know that there is perforation periontintis (Ulcer) in stomach

we wanted to know what would be the cause of perforation, is the acitrom tablet is the cause of perforation or something else.

kindly help?
