post op repair hiatus hernia

4 weks post op and real problem swallowing, just had barium x ray and definate problem, opening too narrow, dr investigating the possibility to dilate with endoscopy, if not will have to undoo work and I will be left as before with hiatus hernia and all its problems.
is this the fault of the surgeon ?
