series of symptoms

I was diagnosed with IBS after a series of hemorrhoids and stomach issues. However, recently, I have been having very specific and unique issues for which I saw a GI specialist. I have a combination of skin tags/hemorrhoids and a lot of pain in my anal area. The pain is sharp when I have a BM, although not always, and it randomly comes and goes throughout the day. The Digital rectal exam was almost unbearable due to the pain. The GI dr first hypothesized there would be fissures but there were none. I had a colonoscopy in 01/08 and there was one polyp (adenoma) that was removed but otherwise clean. The DRE didn't reveal anything unusual or anything suggesting anal cancer (my major concern).

I called the GI dr back to explain that I am still in a lot of pain and he recommended I go to a colo-rectal surgeon. I have an appointment scheduled but I'm not sure what this could be? There has been some random bleeding (once showed as streaks on stool; another showed on toilet paper without a bowel movement but following lots of "cleaning"). My BMs have been okay (occasional constipation but not since the anal pain began). Also, there is an itching sensation. I can't tell if there is also a discharge or remnants from not fully cleaning due to the skin tags.

I am scared to death of what this may mean. I am only 33 but my last gyn exam revealed HPV+ and elevated prolactin. I am also a former smoker.
