You could have an anal fissure or fistual.
Cancer is extremely rare at 20, this cancer occurs after age of 50.
Colon inflammation would not be seen from outside. It's inflammation of colonic mucosa, which has nothing with the outside skin.
It may be thrombotic vein in internal hemmorhoids. I would go to the doc. right away.
I am only 20 years old, hopefully it is not anything like cancer! I do not see any inflamation or sores on the outside, and some days it does not hurt at all..other days it is very painful, but only a bit inside the anus. I don't have any stomach pains or digestive problems or anything like that.
Internal hemorrhoids are sometimes painful if they extend outside the anus, or if the blood vessels are clotted (called thrombosed hemorrhoids).

As a rule of thumb only external hemmorhoids are painful. You need to see your doctor.
Internal hemmorhoids or ulcus from inflammation, or inflamed diverticle, or cancer (if you are over 40-50) could cause the pain.
The only way to find a cause is rectal examination by doctor and then sigmoidoscopy - endoscopy of the last part of the colon.
