when hiatal hernia makes you want to vomit frequently. should it be a concern?

i have a hiatal hernia. and my symptoms feel they have worsen. i always have a urge to vomit and also alot of pain in stomach area,pain in lower back. and also nasuea feeling. should it be a concern?
