10 y/o still poops his pants

My 10 y/o nephew still poops his pants he's been to doctors they've ran test nothing is physically wrong he CAN feel bowel movements, he's been to shrinks they can't seem to pin point anything causing this behavior, he doesn't mind the smell doesn't mind the feeling he's well aware he's doing it, we've tried to make him wash his own underwear and caught him playing with his poop, we've tried taking away electronic's ,toys etc. We've tried the reward system and nothing seems to work my sister is tired of fighting and both his parents have given in and just take care of the problem when he poops as if he's a baby, my sister is embarrassed that she's even covering up for him along with all of this he also wakes up in the middle of the night and pees in his room will not walk to the bathroom which is right out side of his room door not much further from the pee corner in his room, I am also starting to notice he uses every excuse in the book as to why he does this and my sister falls for it, he's got many claims and reasons why he does it none are consistent and im beginning to think he's psychotic and or lazy...horrible to say but really what else can this be?? He's also got 3 brother's, the twins boys are 7 and an older brother who is 14, he's 10 (middle child) and when he's brothers try and talk serious to him about it (who are also tired of his messes) he begins to whine and doesn't want to hear it or brought to his attention...is this a psychological issue or what??

I would for sure focus on the mental health angle on this one. .. it also sounds like maybe it's pure rebellion.

Well ridiculing him about it and scorning him is going to make him feel ashamed of his body and himself, especially if it is of Psychological origin. Has he ever had Neuro-Pysch testing done to test for Autism Spectral Disorders perhaps even Aspergers? Does he have any other chronic health problems either mentally or physically? Anxiety, Depression, ADD, PTSD, Viruses, Illness, Schizophrenia, etc?
