I have a question that I am sure others might like an answer to also.
I have had to collect up all of my medical records for an LTD claim. In doing so, I have found that about 50% of the info on my records is completely made up. What I mean by this is that I ahve gone to doctors and they ahve done a couple of quick exam tests in the appointment but then in their record of what was done, they list a ton of things that maybe they dreamt they had done, but they really had not done.
Also, in an appointmnet, I will have given a description of a problem and told the history of when it started and what caused it an such, but then what the doctor writes down in his notes is totally from fairy tale land.
Because I am trying to fight out a disability claim, it makes things very difficult when my records from various doctors are conflicting because they made up most of the stuff. For instance, I have a primary care doc and 6 different specialists. the primary care notes are correct, but what insurance company is going to listen to a primary care physician. Also, only 3 of my 6 specialists are even in the ball park on their notes. How do I get this stuff fixed? I feel like asking the doctor if he is thinking of writing fiction for a living and is practicing up on his fantasy skills. But I obviously can't do that. So, how do I get them to correct their notes? Right now my disability claim is in trouble because they based their initial decision on info from a doctor who I stopped seeing prior to the onset of my disability and that doctor was by far the best fiction writer of them all. Also, he never even checked me out at all after my first 2 appointments with him. he made up his mind about my diagnosis and refused to change his mind and because he had already made up his mind, he didn't figure he needed to do anything doctory that might help him get a correct diagnosis.
So, does anybody have any experience with this kind of thing? If so, were you able to get your records corrected to make them reflect reality? If so, how did you do it? IT would be very helpful to my disability claim if my medical records were a little or a lot more accurate.
