DeQuervain's Tenosynovitis

I was diagnosed with Dequervain's 6 months ago. Mine is located on the thumb side of my wrist. There is a big bump there and it's very sensitive to touch. It is mostly painful when I wake up in the morning, I can't move my wrist without extreme pain. As treatment my doctor gave me a wrist brace. It hasn't helped much. I'm afraid to get any steroid injections because I am a nursing mother.
How long should I let this go on before considering surgery? I've been hoping so badly it would just heal itself.
