The Emergency Room at the hospital is for emergencies.
A lot of the time the Emergency Room is full of people who have a problem that is not an emergency and abuse the services of the Emergency Room.
Some conditions are clearly an emergency and need immediate attention. A health problem that is persisting is not classed as an emergency, but still would require medical attention all be it sometimes an urgent appointment with your own doctor. If in doubt, a phone call to the paramedics to assess whether the condition is an emergency is helpful. In the UK we have an out of hours doctors available that can give advice whether the condition is such that warrants an urgent hospital ER visit.
The onset of severe pain, chest pains with breathing difficulties is classed as an emergency. More information can, no doubt be found, on the web with regard to the problems that would require urgent ER attention.
In my doctors surgery, they have put up a large poster giving people information with regard to what needs urgent ER attention and what conditions can be dealt with the pharmacist or the doctor.
A urinary tract infection would not warrant an ER visit. But a urinary tract infection that got worse with severe pain, high temperature, passing of blood and not being able to pass urine with the belly size increasing would be termed as an extremely urgent case for an immediate ER visit.
Difficulty with breathing with or without chest pain is always an emergency. Just to give you a couple of examples.

I think what I would do is to literally go by AMBULANCE(so you do not have to sit and wait) to a whole new hospital, with different doctors and have them see what THEY might think is going on with you!! You can't go on like this! obviously, IT IS NOT NORMAL. So........please, for YOUR own sake......go up to another hospital(a "good" one) and tell them everything(even write it down) along with the medications you take and the dosages and start FRESH with new doctors!!! Maybe then you will finally find out what is happening! Try not to wait. You sound like you truly need help as soon as possible! Take care of YOU and let us all know how you made out....ok? You may have several things going on. I used to have GERD but it finally went away. I use to take Nexium.
Your bloating and burping after playing basketball. You could be swallowing air which will extend your stomach and will cause burping. My ex-father-in-law had that problem. He would swallow air while eating and then would have loud burps. Wasn't very appealing at the dinner table.
You also might have a slight case of vertigo. My wife did. She went to an ENT and was diagnoised with vertgo. They did the Eply Manuver on her. It helped. Had to have it done a second time later and has had no further problems. This procedure puts the little crystals that are in your inner ear back into place.
Although anxiety and stress can give you the symptoms of dizziness as too can stomach problems, there are other reasons for feeling dizzy too.
Blood pressure that is too high or too low can cause dizziness, urinary tract infections, ear infections, having problems with your neck.
As you are also experiencing weakness in the arm and heart problems have been ruled out, I would suggest that you should get imaging tests done (xray) on your neck. Arm pain and/or weakness can be referred from the cervical area of your spine. You may benefit from seeing a chiropractor, osteopath or a physiotherapist.
As basketball is a physical game and you will, know doubt, have been knocked about and may be fell down a few times, it is highly likely that there may be an issue with your spine and especially the bones in your neck. Problems in this area can cause balance problems and can cause postural hypertension.
A psychiatrist will help with regard to depression, but will not help to resolve any problems that may be related to your spine.
If you have not had your cervical bones (the bones in the neck) checked out, I suggest that this would be a good idea to get done. Sometimes the bones can get out of alignment that can cause some of the symptoms that you mention.
Some of the symptoms that you describe can be related to depression.
Forgetfulness and memory problems can be signs of depression, urinary tract infections or lacking in Vitamin B12. A doctor would be able to take a blood test to check your Vitamin B12 levels. This vitamin is found in meat, fortified cereals (you can find more information on Vitamin B12 on the web).
