Is there an air bubble in my big toe?

I stepped on a nail seven days ago.  The cut on my big toe never healed properly.  I saw a doctor and received a tetanus shot, but the cut persists, and the skin on the top and 'pad' of the toe continually cracks and peels. When I touch the top of the toe, the skin needs to be pushed until it reaches solid tissue, and when I release my finger, the skin almost springs back up. It's not solid like my other toes; it's as if there's an air bubble on the top of my toe. There's a red crust along one side of the toe nail.  I live in a village with one doctor who is on vacation and I don't know what to do besides keep dousing the cut in peroxide and airing it out.
