He sounds sort of autistic but I am no expert. Have they done medical evaluations? The comment about a neurologist is a good place to start. I hope it can be helped with meds.

He talks to himself a lot and he still buys toys and plays video games all day long. Imagine a 30 year old man that has the mind of a 12 year boy. He will walk up and down the drive way talking to him self and has to be told do just about everything. It's almost like he stopped stopped growing mentally.
What types of symptoms does he have? Try to be very specific.

I would suggest primary doctor & getting them to order some tests to be done & to recommend a neurologist or other specialist in that area.
Some hospitals have programs to help people who don't have insurance. It helps cut some of the costs. I would check with your hospitals near by & see what kind of help they offer. Also, is he on disability or does he work? If he's on disability he should be able to get some medical benefits from it.
