What is the problem with my health.

My LDL cholesterol level has been detected very low. It is as low as 13mg/dl. Apart from this my HDL is 19mg/dl. My triglycerides level is too high to the extent of 1131mg/dl. LDL test was done by the direct method. Fasting sugar is 118mg/dl and the total cholesterol is 176mg/dl. My Hemoglobin is 17.1 gms. Vitamin D total is 6.6 mg/ml.
I am suffering from severe constipation. In the above situation whether any of the deficiency or efficiency is causing constipation. Why my LDL is too low and why the triglycerides are too high. What precautions I require to take. Which food I should take and which food I should avoid.

Good day. I read over your results and these are my suggestions for you.
1. regarding your abdominal ultrasound and pelvis: The prostate is enlarged somewhat  but this can happen due to a variety of things. I would consult with your doctor on this. Inflammation can cause this. If you were older then I would say it is age but you are young. The scan seems to show you have fatty liver and this usually can be due to several things: 1. alcoholism. 2. obesisty.3. diabetes. The liver can feel painful or not and be enlarged. If you drink you have to stop. But you do need to look weight going by your BMI. But we discussed this already. Try to at least get 30 minutes of walking in 5 times a week. It would be good if you could change your diet to the one I mentioned earlier. At least don't eat any added oil no fried food cook without oil. If you could change the diet and have no fat in it as suggested you can reverse the fatty liver problem. The fatty liver is due to a colletion of excessive triglycerides and other fats in liver and is not harmful in itself but the cause of problem has to be removed. The causes being the ones I just mentioned. Now, I looked at the results of your lab work and it looks like your are diabetic so that may be the cause of the fatty liver and high triglycerides. So if you are type II diabetic (you didn't get it from birth) it can all be changed from diet). Some people take Metformin and others change their diet. what you rather do? Instead sticking your finger for testing your blood all the time and having the higher risk of many diseases you can change your diet. It is worth doing. It is hard to start with but my husband and I did it so I think you can if you want to. You have to want to. So, I leave that up to you. I gave you the resources. You can always ask me questions. You can get your blood retested. If they have a insulin tolerance test that might be good to take too. Or to repeat it to make sure before you get on meds or change your diet. Talk to your doctor about it. So your Fasting blood sugar was normal but the one after eating was not. So that shows that you can't get your insulin to drop the level back down to normal.  The Triglycerides were high. So I would normally say either go on the diet and try that or go on a drug called Lipitor (generic is atorvastatin) but you can't take a statin drug because your liver is too bad to take the drug. So you have to do it by natural means. That is why I am urging you to try diet form of change. They may have some other drug that lowers your triglycerides and raises your HDL without harming your liver. You have to ask about that.  Your LDL looks good, the HDL could be higher, the conjugated bilirubin is ok and the gamma GGT is very high as you said.  This is due to the liver disease. There may be something other than fatty liver going on too you don't drink do you? do you take phenobarbital for seizures? This can make the GGT go up falsely, also if you had a biliary obstruction it would up. It could just be the fatty liver which causes cirrhosis.  So you see how important getting that triglyceride down and changing the diet and also maybe getting on a drug for the triglycerides if you can but I dont' know if there is one available if your liver can handle it. The VLDL was slightly high that is just part of the cholesterol fractionations, The creatinine was ok and the Eosinophils were only slightly high. Let me know what you think or if you have any questions.

Helo dear,
I have given the readings only where I found there is abnormality. If you want I can give other readings also. FPG and FLPG stands for FASTING PLASMA CLUCOSE and POST LUNCH PLASMA GLUCOSE that related to sugar test.  My weight is around 85kgs which is equal to 187.39 pounds. weight may be little here and there because I checked it around one month back. I may inform it exact as soon as I check it again.
Thanks.

Hi and sorry I was gone this weekend. I'll have to take a look at some of these results tomorrow. I have to catch up with some stuff and get back to you tomorrow. Keep in mind that laboratory normal ranges vary so I can tell you what I think is normal but they might be slightly off from where they tested you. I do have a couple of questions.
I do know your Cholesterol looks great. HDL is  a bit low. Hemoglobin is fine.
1. What is your weight in pounds please?
2. I don't know what the FPG/FLPG test is for or what the letter are short for?
thanks and I"ll talk to you tomorrow.

Helo dear
I am writing the same message again as there were mistakes.
I had recently underwent certain tests. I am reproducing important findings of the test. Kindly suggest me what is the progress from the earlier situation.
1) ULTRASOUND OF ABDOMEN AND PELVIS
FINDINGS:- GRADE II PROSTATOMEGALY WITH ENLARGED MEDIAN LOBE/MODERATE FATTY CHANGE LEVER
2) FPG 107/PLPG 187
3) TRIGLECERIDES 233/TOTAL CHOLESTEROL 159/LDL 68/HDL 44/ VLDL 47
4)LFT-A CONUGATED BILIRUBIN 0.2 / GAMMA GLUTAMYL TRANSFEREASE(GGT) 234
5) CREATANINE 1.4
6) ESONOPHILS 11
Other reports appear to be normal
Kindly inform what these figures speak as compare to earlier. I am a sugar paitient but the sugar is in normal range. What about this GGT why is it so high ie 234. Is trigliceritedes are under normal range. What is your suggestions for me now. I am continuing the same medicine so far which I was taking. I was taking medicine for cholesterol/sugar/blood thickenning and for strength. I was taking daily 75 mg ecosprin tab for reduce blood thickening now my hemoglobin is 14.8. My weight is still 85 kgs. Please suggest what shall I do.

Hi, Here is the Dr. Mcdougall online discussion board and also a start up guide to diet. But I strongly suggest you get the Starch solution book to help you understand how to be balanced.

http://www.drmcdougall.com/forums/index.php

1. From your results your TSH results shows your are hyperthyroid. So you need to talk to your doctor on how to lower that.
2. I have that your weight is 89kg and the height is 171 cm that equals 30.4 BMI. That is not good if I have your values right. It indicates you are in the obese range. But the good news is that this diet if you are very strict with it will help you lose weight and not be hungry and you have to stay on it for life. It will lower your cholesterol, your triglycerides and so forth (keep on your meds). Later you can lower your meds once your level is down.
3.You diastolic or lower blood pressure is a little high. the 110 is ok the 90 is high but you can get it down . My husband had the same problem and this diet lower it 20 points. The dizziness may be the bp or it may be from the blood sugar. It will go down once you lose weight and once you change your diet. I hope I can help you through this change. I will be happy to. Amazon has the book you can order. But check out the website. I am not a sales person I am a microbiologist who has a back injury and tendonitis and a frozen shoulder so I'm not working. So, I dont' want to be pushy but I believe in this diet because we have been on it for a year and it has help us both. Be careful when walking to start out with. I would walk not jog and just walk and don't get too out of breath. You should be able to talk and walk. Extend your time little by little and later when your bp gets better and you feel better then you can speed up. It will take some time.

Thanks a lot for your replies. My hight is around 171 cms ie 5.61 feets My TSH was tested on 12.03.2013 it was 1.45 uIU/ml(normal range 0.30 to 5.00 uIU/ml) it was done by CLIA method. Urea was detected at 23 mg/dl (normal range 15 to 40) done by KINETIC UV UREASE Method. Creatanine was 1 mg/dl(normal range 0.9 to 1.4 mg/dl) it was done by KINETIC JAFFE (compesated) method. My BP was 110/90 as per the recorded reading on 05.03.2013. I get giddiness only on account of delayed meals. I have told my wife not to use more oil in cookings. slowly we are shifting to whole cereals. I am in search of wheat which is polish free and rice without polish. In cereals we get mong dal with its upper cover other dals we  i am trying to get them. Here in india most of our people eats polished rice. I will read in detail the MCDougal food programme and will follow the same in due course. One thing is of concern whether any of my test readings effect my sexual life. I dont drink alcohal. I am habitual of tea and sometimes coffee but that also I have to forgo because of my constipation. Tea makes my intestines hard. specially in summer tea effects a lot. I will go for all the tests after few days then I will inform you the latest readings. If you like to know any other readings pl inform me. Please let me know about my weight as compared to my height. I feel i should get reduced upto 69 kgs. I have started taking carrots with every meals specially in night I am eating five to six carrots. yesterday I got bowel with little diffculty. I hope tomorrow i may get it some more free. I had a brisk walk also during evening. I am getting  tired fast so i will increase the time for walking slowly. I am drinking    cumin seed boiled in water mixed with lime early morning. Please suggest me any further action. I will be in touch with you.

Well, like you said perhaps the calculation is off due to your triglycerides being so high. I would strongly suggest you try the McDougall diet. It means a lifestyle change to no fast food, no dairy, and lots of veggies and some fruit and lots of starch (potatoes , whole grain yams, whole grain rice etc.). you should lose weight on this diet but it should also lower the pre-diabetis. You may want to also take fish oil pills with DHA and Omega 3, that will bring up the HDL. The ecosprin is a statin drug that the doctor is using to get the cholesteroi and triglycerides down and the shelcal is a vitamin D and calcium pill. Maybe you can ask him or your family practice doctor if you can have your TSH tested because the triglycerides can go up when the TSH is low (hypothyroid). Of course limit your fat and sugar intake. Limit or don't drink any alcohol. It will affect the liver and since you are on a statin drug that affects  the liver too. I don't know if you are overweight it depends on your height. You want your BMI to fall to around 20-21. So if that is high it needs to come down. Give me your height and I"ll calculate it if you wish. Is your blood pressure high too? If they haven't also checked your blood to check your kidneys, and liver they probably should after you have been on the statin for a couple of months. If you eat lost of veggies and high fiber (lots of whole grains) and exercise, you should get better with the constipation. If not then you should talk your doctor about that too. If you have any more questions feel free. My husband had high cholesteroi and High LDL and high blood pressure and he was 210 lbs (overweight). With this diet he has lost 30 pounds over the last year. His blood pressure is now 120/70 (good) and finally his cholesterol is 150 and he is on half of his pills now. I also went on the diet and my cholesterol was 199 and it is now 150. My bp was always good. So we are sticking to the diet despite the comments from our family. There is so much research about it now or I wouldn't have gone on the diet. I read every book I could get my hands on about it. I know  that it is worth the commitment but we didn't do it overnight.
take care,

Thanks for your concern.
My weight is around 89 kgs. I will have to check the weight constantly. I had consulted cardiologist. He ruled out any present problems with the heart. However he was surprised to see my LDL level. He was telling that this is the first time he has seen such low LDL. Immediately he contacted the concerned lab. He was told that the LDL test was done by the direct method because in case of high triglycerides (more than 400)the formula will not work. Even I am surprised because I have underwent all these test after a long time. I never felt any major problem. Suddenly I came to know that I also have sugar on border level. I used to feel proud of being non diabetic. However before consulting the cardialogist I started with Calcikind sachet(for vitamin D) once in a weak. Tab Storfib(Atonastatin calcium 10mg  and Fenofiberate 145mg tablets) one tab daily night. Tab Glychiphage 500mg daily once and Ecosporine 75mg once daily. After consulting the cardialogist he chaged the tablets to Tayo 60K one per weak, Tab Perzeek one daily for six weeks, Tab tgr 160z one daily, ecosporin 75 and shelcal 500 one daily. I am trying to avoid oily foods as far as possible. Taking much water. But my constipation is troubling a lot. During summer it will be much more. I will have to try one by one tricks how to avoid constipation. I was taking luke warm lime water during night but it worked for two days now it is not working to that extent. I have started brisk walking also.

Something is wrong with your lab values. If the triglycerides are 1131 and the LDL is 13 and HDL 19 you calculate the cholesterol by Triglycerides/5 + LDL+HDL. So your Cholesterol should be around 258, /are you sure those are the correct numbers? What is your weight? With the triglycerides that high you really need to see a good doctor. But you can start with a good diet. Another good book you can take a look at is from the Exsalus Health and Wellness Center called Keep it Simple, Keep it Whole, your guide to optimum Health by Alona Pulde MD and Matthew Lederman, MD.
Good luck. Let me know if you have any questions getting started.

I disagree too that there is plenty of fat in fruits , vegetables and whole grains. You don't need saturated fats for the body to function properly. That is an old way of thinking. There is so much data to show that butter is not good for you.

Sorry I misread that your cholesterol was low too. So, I think it is ok to go on the diet. The LDL being low is good but I am asking the Dr. McDougall site if the LDL being so low is any problem. I don't think so but I'll get back to you. In the mean time drink lots of water and eating healthy, lots of fruits and vegetables and whole grains , legumes, like lentils, beans etc. rice , yams, or sweet potatoes, green beans etc.and starches , no fats, no butter no oils except fish oil because it is hard to get DHA in your diet and you get too much fat and other things from fish and flaxseed or flaxseed oil is good too. You have to read it for yourself.

I had to add, butter is terrible for you. I strongly disagree with the other post about that. Read the book I suggested and watch the video Forks over knives (video is on line) All butter is good for is clogging your arteries and making your triglycerides go up more.

I still think you should wait to do anything (diet, or clease) until you make sure you don't have a blockage or a big reason your LDL/HDL are so Low
If you go on the vegan diet your cholesterol will go even lower. So, I would really see a doctor first if you can.

Yes you are right your LDL is too low the HDL is too low the triglycerides are way too high, and the Vitamin D way too low. The Hemoglobin is normal for a man  and the fasting blood sugar is pre-diabetic. Of course these values may be somewhat different depending on the ranges at your laboratory. At your age I'm not sure why your cholesteroi and lipid levels are so off. It may be due to bad diet or genetic factors or malaborption of nutrients. But because your triglycerides are so high you really should see a doctor for this The severe constipation could potentially be a blockage though. So you may want to get that checked first and if you have no blockage. Ask your doctor how much vitamin D to take. You may want to start taking 1000-2000mg a day for 3 months and get you blood tested for vitamin D again and see if it has gone up. you want it to be about 30-50mg/ml. You may have to go on a drug such as Crestor or one of the other statin drugs to get your triglycerides down and raise your HDL. After they get your LDL and cholesterol up and triglycerides down or find out the reason for this then you can change your diet and I would recommend a vegan diet without added oil. The you will still supplement with vitamin D, fish oil with DHA and Omega 3 and only once you have been vegan (no meat, fish of any kind) for 3 years then you can take a supplement of Vitamin B12 once a week at 500 micrograms a week or methylcobalamin or hydroxycobalmin NOT cyanocobalmin.  So, there is some investigating to be done first. Get a lower GI test or a colonoscopy whichever they can do to check for a blockage. Then, you need to find out what the doctors may think is causing the high tricglycerides and low cholesteroid, and LDL and HDL. They may give you drugs for this to lower the triglycerides and up the LDL/HDL. If you can't get any of this done you can start on the vegan diet. I can give you a reference to a book you can get on online it is called the Starch Solution by Dr. John McDougall. MD. He also has a website. But basically you can eat lots of starchy foods like potatoes, whole grains, and rice etc. and then you have lots of vegetables and some fruit. About half starch and half vegatables and 1-2 fruits a day. You can have some soy products if you like. So the Indian diet is a good diet but no meat, and no oils. You have to learn to cook without oil so you subsititute adding water into the frying pan etc. See the book.. Drink lots of water (8 glasses per day). If you have tea or coffee you can cut one or two glasses back. Hope this helps.

Thanks for replying
How about castor oil in place of coconut oil. Its difficult to eat coconut oil as it gives strange smell. I was told that persons with low level of LDL  will be prone to cancer, is it true. What multivitamin supplement I should take is it in foods or in medicines. Please suggest any food preparation which contain multivitamins.

Start with a cleanse - Epsom salts in a glass of warm water.  Then be glad your LDL (Lousy cholesterol) is low and your HDL (Healthy cholesterol) is high.  As for the triglycerides - eat more healthy by avoiding fried foods.  If you must fry something use coconut oil.  Try eating more fruits and vegetables to keep the bowels moving and maybe a multivitamin supplement.  We all need vitamins as our food has been tampered with and no longer supplies us with the nutrients we need.  Remember that butter is good for you, too.
