pulse sound in the ear

I am 18 years old. My problem started last year. One day I noticed I had ringing in my ear like you hear a ocean in your ear. After much irritation I finally went to my docter and she said I had fluid in my ear, which made sense. After taking nasal spray and dicongestant medicine it got somewhat better. Eventually it just got worse again, because I was having pain just on the side of my head where my right ear was the problem, dizziness, and hearing loss. So I went back to the docter thinking their was still more fluid. She looked and found no fluid but concluded I was hearing my pulse in my ear. She suggested I do a CT scan which I did 3 days later and it came up normal. But the problem has persisted. Pain shots through my neck and that same right side with my ear irritation. Is it a allergy? Some form of tinnitus? It has caused me pain and loss enjoyment in my last few months of school. What are my options?
