I need to know what this means, please help.

i had an ultrasound done because I have been having a lot of UTI's and very painful and irregular periods. I do not know what the results mean. The NP is terrible! I also suffer from hashimoto's thyroididtis.


Transabdominal and transvaginal ultrasound of the pelvis

Indication: Enlarged Uterus

The uterus is normal in size and appearance with a thickened endometium. The endometium measures 11.0 to 13.0 mm thick. The uterus measures 9.1 x 5.7 x 4.4 cm.

There is a 1.7 cm right ovarian follicle. The right ovary measures 3.2 x 3.0 x 2.4 cm. The left ovary measures 2.6 x 1.7 x 1.6 cm.

Impression
1. THICK ENDOMETRIUM.
2. A 1.7 CM DOMINANT FOLLICLE INVOLVING THE RIGHT OVARY.

Your uterus is at the upper limits of normal size (about 1/2 inch bigger than average).
The follicle in the right ovary is normal--means you are going to ovulate from that right side this month.  The thickened endometrium is where the problem is.  You probably have a build up in the uterine lining (endometrium) from irregular ovulation/menstruation.
Irregular periods tend to be heavier, clottier and more painful.  If my patient had these results, I would give her a 14 day course of Prometrium (natural progesterone) to clean the lining out and get rid of the excess tissue.  (If she was over 35, I would suggest an endometrial biopsy first).  Then I would discuss with her how to keep her periods regular and/or keep the lining thin.  One option that a lot of people really like is the Mirena IUD. Obviously if you are trying for pregnancy that is not the way to go though.  Another option is birth control pills, or monthly progesterone.  OR if she was "trying" I would start Femara to help her ovulate.

Thank you very much! I guess I can tell you that I have 3 children and had a tubal ligation after the birth of my daughter. I am 32 years old and yes all of the symptoms you mentioned are me! The heavy clotting, heavy bleeding and the pain is WOW! Very painfull! I saw a Nurse practitioner and she told me that I was just fine and she let me go. I started looking into this because I have to see and endocrinologist for my Hashimotos and I have a nodule on my right lobe with Hurthle cell change.
