Merina or Copper IUD... or neither!

Breif history:  I am 37, onset of menses at age 9, severe dysmenorrhea, heavy bleeding with clotting, not started on OC till age 21.  G1P1, 9lb 11oz baby, grade 2 rectocele/cystocyle.  Currently I have tried just about ALL OC with main SE being migraines and HTN.  I am now taking Lisinopril 20 to combat the HTN.  Not over weight 5'6", 125  body fat 16%, BMI 19.  I have had multiple ovarian cysts rupture, chronical ovarian/pelvic pain, several small fibroids.  I have had an endometrial BX and D&C, negative. Never had laproscopy (GYN is suggesting it to rule out endometriosis).  Contraception is NOT may main concern!  Controling my painful heavy periods is the main issue while not giving me horrible migraines and HTN.

My GYN is suggesting a hysterectomy at some point but until then a Merina IUD.  The forums I read about this product scare me!  I read about weight gain, elevated BP, seere cramping, acne, back pain... ect.   I do not feet his is a good choice for me.  Whay would a copper IUD not be a better choice?  Not ready for a hyst as I may want another child.  Only have 1 now.

My experience with the Mirena has been a very good one.  Because it releases hormones only locally to the uterus, women who use them have very few side effects.  I have not had any patients who experienced the horrors that you have read about! It sounds to me like some of the people who are writing may be confusing the Mirena with Depo Provera--those are definitely side effects of the Depo shot.

In fact, the copper IUD has been a bigger cause of problems than the Mirena in my clientelle. It tends to make period cramping worse, And the bleeding heavier! Women who like it, really swear by it though.

The Mirena, in my experience usually lightens the flow tremendously--or even stops the bleeding altogether. Cramping tends to subside with the bleeding.

Have you considered the GnRH agonists like Depot Lupron?  If you do have endometriosis--you might experience tremendous symptom relief with use of these, and it would help to keep the disease from advancing until you are ready for your next child.  Its difficult to know how it  would affect your migraines--some people get better, others get worse.  It does NOT provide birth control.  It does stop your periods. Other side effects include hot flashes, and mood swings.  It should not worsen your blood pressure.

All things considered, I have to agree with your MD that the Mirena sounds like a really good option for you.  If you don't like it, you can always have it removed, and within a month you are back to your normal baseline periods and fertility.

When the time comes, you are going to love your hysterectomy! Be sure you don't do it UNTIL your time comes--you will know.
