Dr. Hunter, Do I need psychological help?

Dear Doctor Hunter,

In the last 8 months, I had have sex with 12 prostitutes.
(September 2007 ->
December -> 2
January    -> 3
February  -> 2
March      -> 2 (March 17 2008 after in June 17 2008 I got tested HIV - in two different laboratories)
July         ->1)


Facts:

- Always protected sex, anytimes unprotected oral sex (receptive),
- With a little razor burns in my pubic zone (by the shave) in two times.
  (In the mourning I shaved my pubic zone) and  burns me with fluids (sweats or vaginal fluids)..
- One time condom broke (September 2007) one minute of exposure.
- In June 19 2008 I got tested in two differents laboratories being HIV - both .
- The last exposure, July 19. (protected the condom didn't broke, (I filled the condom with water and the water didn´t scape of the condom - kisses, and I perform oral sex to her indirectly because she passed her fingers by her vagina and after put the fingers with fluids in my mouth)


Questions:


- Am I in a high risk level for acquiring HIV?
- Should I be tested newly in three and six months?  
- If all the prostitutes was HIV+ am I in a serious trouble?
- Do I need psychological help for leave the sex with prostitutes (I am a single man, without girlfriends, but I want change my life, because I have too much guilty feels).

It seems as though you got an HIV test over 6 months after the broken condom. An HIV test after 3 months is over 99% conclusive; meaning that at 6 months you can be sure to be HIV negative. Protected intercoarse is a no risk exposure. Therefore, since you were tested for HIV after the broken condom, and have properly used condoms since the HIV test; I am pretty sure you can relax in the fact that you are HIV negative. I don't think the doctors in this forum can assess whether or not you need psychological help; as they specialize in HIV and STD'S and not in psychology. I would suggest seeing a psychologist to answer that question.

sounds pretty safe to me with the exception of the broken condom during vaginal sex.  having oral sex performed on you and one time cunninglus pretty much zero risk.

The only exposure for which HIV has not been ruled out by the testing in June was your exposure in July.  In July the vaginal exposure was protected and the indirect oral exposure has no risk for you.  Thus you have no risk and do not need additional testing.  As for your sex with prostitutes, I cannot answer that but that you even asked it suggests that you might benefit from talking from someone.
