Dear Dr. Handsfield and Dr Hook,

I am a reasonably intelligent person and I posted my question in the "prevention " forum before.

I had oral sex  unprotected without ejaculation and  receptive anal sex. We discussed HIV status beforehand and I had relations with this person 4 years ago. I am bisexual and every now and again the urge is overwhelming!  He claimed to be "clean" , but I cannot verify this as he said he was tested one year ago.I recollect him donning a condom and applying lube.

I clearly remember him gesticulating and pointing to his condom covered penis after a position change, assuring me it was still in place and intact.

I clearly remember him removing the condom after the "deed" and handing it to me, upon which I blew into it to confirm integrity. And I clearly recollect him ejaculating on the bedsheet.

My question is of a "mechanical nature"....paranoia as you will.....How likely is it that he somehow he penetrated me bare without me knowing, even though it is highly unlikely. And if so what are my odds of having been infected?

27 days after this event I went to Walgreens in the U.S since oraquick 2 swab test is not availlable in Canada, and I drove 4 hours to cross  border.

Negative!

I am fully aware that there is a current controversy about the ora quick swab test and the results are not out. And I am aware that a conclusive result is not feasible before 12 weeks. I have constructed (lied) several more excuses to not have sex with the person I love the most. I cannot chance infecting her!!

I am bisexual and I have been making up excuses as to why I cannot have sex with my girlfreind. She is getting frustrated and I am running out of excuses.

I've had no symptoms, none! no fever or  enlarged lymphnodes, sore throat... rash....nothing!

Your input would be greatly appreciated.

Welcome to the forum.

Congratulations for a very safe approach to your sexual adventures with other men.  You are apparently using condoms consistently for anal sex, and you are doing your best to assure that your partners are unlikely to have HIV.  And as you were told on the community forum, oral sex carries little or no risk for HIV, especially for the insertive (penile) partner.  If these habits don't change, you could have similar exposures daily for 50 years and expect to never catch HIV.

As for advising whether your partner was gaming you, i.e. going through a charade of condom use while not actually using one, all I can say is a) you were there and I wasn't:  how can I possibly know?  And b) it sounds unlikely.

There really isn't any "controversy" about the oral fluids rapid HIV test.  The scientific data are clear that it takes longer to become positive than the blood tests, both rapid and lab-based.  Your negative results at 4 weeks probably is around 70-80% reliable.  In my view you really don't need testing at all after the sexual exposure you have described -- but having headed down that path, if you want a definitive results, you could have a duo test (for HIV antibody plus antigen) at any time; a standalone antibody test at 6-8 weeks; or another oral fluids test at 3 months.  If you do any of these, you can definitely expect the results to remain negative.

I hope this has helped.  Best wishes--  HHH, MD

Not sure if it's appropriate but it's been 84 days and my last test was negative!

You were completely correct in your assessment. ( I tested excessively at 4,5,6,7,8,9,10,11 and 12 weeks ! )

I just would like to thank you for the advice and putting up with my anxiety.
The weekly testing is something I HAD to do . I hope you don't take this as I doubted your initial advice.

For what it's worth..... to all the folks that post here....trust the Doctors in here and the community members! From reading the forums over the last three months I now realize that they have seen it all and they know what they are talking about.

Again Doc,  a sincere Thank You !

Regards

It is already at least 99% certain you aren't infected.  It will be 100% after the negative result on the 12 week test.

That ends this thread.  It's time to move on.

Hello again,

Not sure if I am allowed to post again but if I have to pay again I have no problem with it.
From what you've told me last time I have to  tell you that I have been hugely relieved.
Be it as it may, I can go through a day now  quite well but the occasional panic attack and overthinking kicks in, but far less than before I consulted with you.
I researched this issue ad infinitum and I came to the conclusion that you know your ( fill in the blank) !...pardon the language.
I have over tested ..i.e at  4,5,6 ,7 and now 8 weeks post exposure( which you deemed not test-worthy to begin with) with the over the counter Oraquick Oral Mucousa test kit.
I bought one extra for the mythical 12 week confirmatory test. Guidelines are guidelines even though I regard your  stance on medical/legal requirements on the 12 week window period as more trustworthy..
I would greatly appreciate your input on a perecentage figure of my negative status knowing that I tested negative at 8 weeks  ( self administered oral swab test) post event.

False positive results are rare with all HIV antibody tests.  This shouldn't be a concern for you.

Thanks for the thanks and the kind words about the forum.

I am a professional in my field and I know you are a professional in yours. You clearly know your "stuff".
I cannot tell you how  much I appreciate your input.
This site is truly a well of knowledge!
I can assure you that condoms will never be a consideration in the future for me as this last experience has told me a lesson. I shall not stray ever again!
If in fact my final test will be negative.
Your knowledge and dedication to this cause is worth every penny.
Thanks again.

P.S...how afraid should I be about a false positive at 12 weeks with an oraquick swap test?
