Please Answer Really Worried

Hi Doctor,

I am a 25 year old Indian male. Last week in Tokyo in a nightclub I had the following exposure:

The nightclub was filled with a lot of prostitutes, which I realized only after staying there for an hour. I have a lot of pimples on my face and unfortunately one of the pimples burst when I was in the club that night and kept bleeding very heavily. While at the bar I reached out to speak to a Thai girl of unknown HIV status (she told me she was in Tokyo visiting with her friend but she may have been a prostitute). The girl was quite short and I had to get my face close to hers and even touched her face with my face for a significant period of time (for more than 2 minutes) while chatting with her. I will be grateful if you could answer my questions below:

1) Assuming the girl was bleeding heavily as well from her face and the fresh blood extremely high on HIV content from her face came in direct contact with the large amount of fresh blood coming out from my pimples am I at risk for HIV transmission?
Please note that there was no other direct physical contact whatsoever between both of us. We didn't kiss nor touch each other's genitals or anus during the conversation we had for about 5 minutes.
2) Assuming the girl was bleeding heavily as well from her face and the fresh blood extremely high on HIV content from her face came in direct contact with my pimples am I at risk for HIV transmission?

After the conversation with her, I visited the toilet and checked my face in the mirror and there seemed to be a significant portion of blood coming out from my pimple. I am hence worried.

What is your assessment of my above exposure with the lady if very high volumes of HIV infected blood from the lady's face came in direct physical contact for a SIGNIFICANT TIME PERIOD OF 5 MINUTES with the 2 pimples on my face that were bleeding heavily?

So just to be clear, if very high volumes of HIV infected blood from the women's face came in direct physical contact with the 2 pimples on my face that were bleeding heavily, is there any kind of risk for HIV transmission to me?

Welcome to the Forum.  You seem to have some misperceptions about how HIV is spread.  HIV is spread in adults ONLY through penetrative sexual contact or injection of infected material deep into tissue.  In your case you do not know that the woman you spoke with had HIV and statistically, even if she was a commercial sex worker, it is unlikely that she did have HIV.  Further even if she was, no matter how high the HIV concentration in her blood was, the sort of touching and possible surface contamination with blood that you describe would not put you at risk for HIV.

There is no reason for concern related to the events you describe and no need for HIV testing.  EWH

3) I am also not sure if 2 pimples burst and bled heavily or was it just one pimple. Will your assessment change if it was 2 pimples bleeding very heavily as opposed to one pimple?
4) Have you seen or read about any situation before, where HIV was spread from one person to another through the exposure I had?
4) Do I need HIV testing for this whole exposure?
