Question on ARS and Oral Exposure in Dubai

Dear Dr HHH

i have gone thru many threads and i know for sure that unprotected Oral (BJ) carries zero risk, however anxiety plays its own game and hence i was looking at answers to some specific questions.

My exposure:-
date of exposure - 20th April 2013 - she was a turkish escort, claimed to be on a resident visa, premium escort @2000 dhs....worked in a salon during the day and did this for extra money to support family back in turkey...she asked me for a condom and she dint carry one for random checks in dubai by the cops. Anyway she started with an unprotected oral blowjob and then we had protected sex twice...condom was intact. when asked directly she said she was clean, recently tested and bcos she was on a resident visa testing is frequent.

Question 1- she had a lip piercing which she removed during the BJ, wud any blood from that pierced hole put me at risk during the BJ?

Question 2 - i got a sore throat on the 9th day after the exposure with yellow phlegm, i took antibiotics and never had any fever, does that mean the medication supressed the ARS symptoms like fever n rash?

Question 3 - Do i need to test?

Question 4 - what scares me is that bcos i cudnt finish the first time with protected sex she OFFERED me to go without the condom the second time but to ejaculate outside. I insisted on using the Condom and finished up the second time. My mind says she must be this casual with others and she is infected and hence i wud have got infected during the BJ?

Question 5 - I was all fine after reading other posts but when i got this sore throat on the 9th day the anxiety flaired up and hence im writing to you for an asssessment. Can i just forget about this move on and continue with unprotected sex with my wife?

Welcome to our Forum.  As you may know, Dr. Handsfield and I share the forum and take questions as out time permits.  Today you got me.  FYI, the reason we share the forum is because we have worked together for nearly 30 years and while our verbiage styles vary, we have never disagreed on management strategies or advice to clients.

Congratulations on your use of the search function on our site to assess your risk.  We appreciate it and encourage our clients to look at other posts on our site for information.  As you have already determined, the exposures you describe did not put you at risk for HIV.  Few if any CSWs in Dubai have HIV and neither your receipt of oral sex, nor your condom protected sex placed you at risk for HIV if she did. You need not worry and do not need testing related to this exposure.  

In answer to your specific questions:

No, her lip piecing does not change the risk in any way.  Still no risk.

Question 2 - i got a sore throat on the 9th day after the exposure with yellow phlegm, i took antibiotics and never had any fever, does that mean the medication supressed the ARS symptoms like fever n rash?

Antibiotics have no effect on the ARS.  Your sore throat occurred sooner than would be anticipated if it were due to ARS and your did not have other symptoms of the ARS.  Your symptoms do not suggest the ARS.

No,

I don't blame you that her behavior suggests that she has had unprotected sex with outers. Even so, that does not mean she is infected (most CSWs are not) and does not change the fact that your exposures were no risk for HIV.

Your sore throat is a coincidence.  I would not worry and see no reason related to this exposure for your to worry about unprotected sex with your wife.

You should have put this behind you before as the exposures you described did not put you at risk.  Your test results add further proof that you were not infected with HIV or STDs.  

I said the thread would end before and chose to answer this further question. There will be no more answers.  EWH

dr because of my anxiety i went ahead and tested for a full panel std which was negative and specific to HIV i took a HIV duo 4th generation test for antigen and antibodies at 3 weeks negative and HIV RNA for HIV 1 - Cobas Ampliprep/Taqman - Qualitative at 3 weeks negative.

can i just put this worry behind me now?

I presume/guess that there are so many questions about oral sex becasue it is so often unprotected and, despite sceintific evidence that it does not occur, peole have a hard toime bleieving HIV is not spread through recipt of oral sex.  

As a generalization for both you and other readers, you must realize that we VERY frequently get questions asking if different types of exposures or prevention measures are 100% effective.  The answer to that is that this is scientifically impossible.  For a variety of mathematical reasons far too complex to go into here, all one can do with well conducted scientific studies is estimate probabilities.  By definition, any estimate, cannot ever be 100% certain because of the incredibly low possibility that someone will get HIV through a previously undescribed mechanism tomorrow.  On the other hand, when Dr. Handsfield or I say that something is virtually zero, very close to no risk, or of minimal risk, or use any other term to indicate a very small risk, that means "close to zero" in a world where zero cannot be attained.

If Oral sex is safe sex, zero risk, no risk why is there so much scare and anxiety out there with people and so many questions on oral sex on the forum.

What is triggering questions on oral sex again and again and again?
