Risk assesment + picture

Hi guys hope you're both well,

I'd like to make it clear what Im asking is nothing to do with my previous concerns. I just want a risk assesment and opinion on the pic.

Went Ibiza on a stag do. Lots of beer. Passed out one night. Had a feeling Id had sex (spiked maybe) when I woke up the next morning I had a girls phone number and was minus my underwear. I have since text her, she assured me it was protected. Then I made a mistake of guessing her hair colour based on her profile picture on facebook- two freinds, I was totally wrong. She told me I had the worng person, not heard back since.

I got a text from an unknown number the next day claiming to be a Kenyan. I think it was her getting level with me- the no was +44 (uk dial code) and was back  in england a week later.

5 days: 2 white marks on lips, went after a day
14 days: Diarreah for 2 days + fatigue
28days: Possible swollen lymph node in groin

I had a duo at 6 days which was negative. Im at 30 days now, going for a retest on wednesday.

1.What would you say my risk was?
2. Are my symptoms concerning with regards to HIV
3. Does that look like a lymph node in the picture? (I went to doctors only last week, worried about going back again so soon)
4. Intially I was very confident I was low to no risk. After my intial run of stds came back clean I was confident I must have used protection. Now I am worrying again. .. My wifes been fine..

Directly to your specific questions:

1) It isn't possible to assess your risk of HIV exposure, since you apparently are uncertain whether you had sex; if you did, whether or not it was condom-protected; presumably you also may not know whether it was vaginal, anal, oral, or some combination of these; and you say nothing about the context, such as partners' nationality, their known HIV risks (commercial sex? injection drug use?); etc.  However, assuming a single episode of vaginal sex, even if the woman had HIV, the average transmission risk has been calculated at 1 in 2,000.  So that's probably your maximum chance of having acquired HIV.  (To put that in context, it is equivalent to having unprotected sex with HIV infected women once a day for 5 years and maybe never catching it. This is part of the reason why most husbands/wives of HIV infected partners never catch the virus themselves.)

2) Your symptoms don't sound like those of a new HIV infection.  Among other things, acute HIV doesn't usually cause diarrhea, and ARS-related lymph node inflammation occurs body-wide, not just in a localized area like the groin.  Anyway, even when someone has classical symptoms, usually things other than HIV are the cause.

3) We do not examine posted photos on this forum.  In any case, a photo cannot tell whether a lump is a lymph node or something else.

4) Your initial confidence probably was justified.

The negative test at 6 days was meaningless; that's too soon even for the Duo test.  But you're about to be tested -- that will tell the story.  If it's another Duo test, a negative result will be definitive.

You have a track record of anxiety-driven reliance on a number of MedHelp forums, including the necessity of a recent warning on the community forum.  Let's limit this thread to a single follow-up comment after your upcoming repeat test.

Thanks for the heads-up.  I was unaware but have now looked at the journal articles themselves.  The science seems excellent -- far beyond my technical expertise, but papers don't get into the journal Science without being very high qualilty.  It looks like a promising step on the way to a potential vaccine, but still a very early one.  There are always big potholes from this level of discovery to making an effective vaccine, and HIV science is full of disappointments along the way.  If a successful vaccine comes from this work (I certainly hope it does!) it is several years away.  Those interested in HIV prevention should keep an eye out for further developments, but don't hold your breath.

Thanks for your support, by saying the GUM was superb I wasn't being disrespectful towards you. The level of support they provide pales into comparison against what this forum does.

I'm sure you hear it all the time, but you guys are absolutley EXCELLENT. The Time you take to answer each individual question (even though they are repeats of repeats) is exceptional. I applaud you- top top guy!

The test proves you don't have HIV.  I don't know the cause of your red spot and will not try to guess; it has nothing to do with HIV.

Since you are now under the care of a supurb clinic, that is where you should direct any additional questions.  They know just as much about HIV as I do, probably more.  Most likely they will agree with me that you do not need any further testing.

That will be all for this thread.  Stop worrying about HIV.

Hi Doctor,

Since I last posted I have developed a small red spot, not in the arm pit, but on the inside of the bicep right near the armpit very close to crease, but not quite in the crease. Im guessing its just an ingrowing hair follicle/cyst/boil etc..

I rang the NHS clinic today (they are superb and I am proud England boasts such superb testing facilitys and support). My test which was an HIV DUO, returned negative, this would have been 31 days post exposure. Would you say the is me in the clear, conclusively negative- no need for repeat testing?

She was white female from Manchester England- Poss on holiday, maybe working there- I dont know if she was a prostitute but I have never been with one and dont think id have started evem paralytic or drug induced so Im discounting that. She did assure me it was protected and she didn't have any STDS which held up at 6-7 days. We shall see next week.
