SLEEPLESS

Hi doctor. I practice abstiance for religious purposes. Last thursday I gave a woman a ride in the street. She gave me unprotected oral sex until completion. This happend on a thursday. By sunday I felt a lower back pain and discomfort in my testicles. I took a shower and I notice my testicle has enlarged. It was hanging so low like a sock with quaters in it. It was big and my sack was red. I thought it might be blue balls so I gave my self "a release" and my sack went back to normal. Well my testicles are still sore and it does not burn when I pee and their isn't any discharge or lesions, but my testicular sack is still red and both my testicles are sore. Well what would my hiv risk be from this exposure? what would it be for other std's. I saw my doctor today (monday) and he felt around there and said theirs no lumps. He told me to pee in a cup and found no bacteria or pus from this little strip he put inside. This has been my only sexual exposure in the last 2 years. Would an Hiv test be required? could another std be causing my testicular pain and tenderness...could it be Epididymitis? what other std risk may I have?

I don't know what it means either.  I have never heard of oral sex leading to prostate gland infections or similar problems, although I suppose it could happen.  I'm still skeptical that an infection of any kind is responsible and predict that the antibiotic will make no difference in your symptoms.  I hope I'm wrong, of course.

You might read up on nonbacterial prostatitis, most commonly known these days as the chronic pelvic pain syndrome.  Google that term for lots of good information; start with the excellent Wikipedia article and also read the information you will find from the Stanford University Department of Urology.

In any case, this isn't an STD.  Follow up with your doctor.  I won't have anything more to say.

I WENT TO THE DOCTOR (BROKE INTO THE SAVINGS ACCOUNT). MY URINE SAMPLE CAME BACK NORMAL WITH NO BACTERIA OR INCREASED WHITE CELLS. MY TESTICLES ARE TENDER AND MY PROSTATE IS BOGGY. HE GAVE ME ANTIBIOTICS (LEVAQUIN 500 MG) HE SAID ITS NOT AN STD, BUT IT MIGHT BE RELATED TO MY ORAL EPISODE. I HAVE NO IDEA WHAT THAT MEANS..

Your symptoms don't suggest any STD, but you could visit your local health department STD clinic for expert care at little or no cost.  Since you don't have an STD, and that's the only topic for this forum, I won't have any other comments. That will have to end this thread.

Doc I don't have insurance and many of the urologists are telling me it will run in the hundreds to go see them. In these past few days my scrotum is only red after ejaculation. Theirs still no pain when I urinate and no discharge. I woke up today with something dry over the head of my penis. It was clear and but I can barely see it, but I think it was precum. The tenderness and dicomfort has gone from my testicle to my two buttcheeks. That space from my testicles to my butt is also tender. I notice they become tendr when I sit for to long or do some type of movement like bending over, cleaning, picking up things etc, etc. Their a little tenderness when i ejaculate. Can I rule out any std as this point?

i did tell my doc about this exposure....i am happy as longest i dont get hiv

Epididymitis is usually obvious on physical examination.  Your doctor is not likely to have missed it.  Also, epididymitis is uncommon in both testicles at once.  However, there are causes of epididymitis other than STDs, and having it on both sides could be caused by certain viral illnesses or drug allergies.  If you are convinced your testicles are tender and/or swollen, you should call your doctor for a follow-up appointment; or perhaps ses a specialist, such as a urologist.

And yes, your pain could be non-sexually caused.  If you do have epididymitis or some other testicular inflammation, it probably is not from the oral sex event, just a coincidence.

Gonorrhea of the penis without symptoms is rare.  If you had gonorrhea, probably you would have noticed the pus dripping from your penis; and the urinalysis would have shown elevated white blood cells.  However, you should have told your doctor about the sexual exposure.  If you did not do that, call and do it now so that proper testing for STDs can be done.

Please return to the forum and let me know what is found if and when you are reexamined by your doctor or have been seen by a urologist.  Until then, there is no point in speculation; a definite diagnosis cannot be made in this forum or by any other kind of online investigation.

So you can tell if someone has epididymitis by physical exam? if I did get gonorrhea would my risk for HIV increase? I took a regual urine exam 3 1/2. Doctor says theirs no bacteria. I know that's not a gonorrhea test, but would it catch epididymitis. I have no swelling now just soreness. My sack was red but its got a little better. My testes are still sore from the sack all the way yo my left buttock.

Welcome to the HIV forum.

There is no realistic risk for HIV from this exposure.  Even if your partner had HIV, the chance of transmission by oral sex (from the oral to penile partner) has been estimated at one in every 20,000 exposures, and some experts believe such transmission never occurs at all.  And HIV is definitely not a possible cause for the symptoms you describe.  From a medical or risk standpoing, no HIV testing is necessary.  However, if you remain nervous about it, you could have an HIV test 6-8 weeks after the event.

It is possible to catch certain STDs from oral sex, including gonorrhea, and gonorrhea can cause epididymitis, testicular pain and swelling.  However, if your doctor couldn't find anything wrong on examination, you shouldn't worry about it.  Both gonorrhea and epididymitis would have been obvious to the doctor.  Other STDs that could be acquired by oral sex are herpes, nongonococcal urethritis (NGU), and syphilis, but any of these would have caused symptoms that you don't have, like dishcarge of pus or mucus from the penis or sores on the penis.

If you remain concerned about these things, or if you decide you want an HIV test, discuss it with your doctor then follow his advice.  In the meantime, don't worry about it.

A final word of advice:  You should consider carrying condoms.  You obviously are having some difficulty following your religious convictions, and it seems likely that someday you will be tempted to have vaginal sex.  If that happens, the risk of HIV and other STDs will be very high if you select partners like you describe above.  A word to the wise.
