Unprotected Sex, 3 scenarios in 1 night, Anxiety, Help!

Hello, Doctors and thank you for the help (even though I’m the biggest retard and I’m paying for it)

Dear Doctors, I need some help, I am a 22 year old male, uncircumcised, no hard drugs, with 1 day after exposure and here is what happened (first time doing this and is was the worst mistake of my life!!!!!):  After living for six months in the city of Cali (Colombia) studying pretty hard and no type of relationships whatsoever, I had an incredible urge to go and have as much sex as I could in one night (stupid me!!!).
I would like to also apologize if my message is too explicit, but I believe that even small details can mark a difference.
1) I began my night by having unprotected sex (mostly oral –vagina to mouth and penis to mouth - with only a few minutes of raw vaginal sex) with a young sex worker. I proceeded to finger her, and of detail was the fact that she felt a little scratch from my fingernails so I stopped. I wore a condom for most of the time, but after mutual consent and reassurances and even though she was slightly reluctant at the beginning (she said, though she could have been laying, that it was her first month doing this type of work) that we were both clean, we did it unprotected, but I could not reach climax. Also, my penis was a little sore (red from the stretching, I’m guessing) at the end of the stretched foreskin, where the shaft begins, though there was no blood or swelling.  She then left.

Welcome to the Forum.  Your clearly regret your actions and I suspect that is contributing to your concerns.  I hope my comments will help to provide some perspective.  You do not mention how long it has been since these activities.  This, of course, influences the issue ot testing.

You mention a variety of different exposures including deep kissing, masturbation (fingering and receipt of masturbation), giving and receiving oral sex, and protected  and unprotected vaginal intercourse.  You do not know the HIV status of your 3 partners but given their professions there is likely a modest risk of STDs being present, as well as a low risk of HIV (most commercial sex workers do not have HIV).  Overall your risk of HIV is low and your risk for other STDs from these exposures is moderate at most.  There is NO meaningful risk of HIV from kissing, masturbation (you or your partner or your partner of you), receipt or giving oral sex, or condom protected vaginal sex.  Unprotected vaginal intercourse is associated with transmission of HIV about once in every 1000-2000 sexual exposures.  If a partner had another, more common STD such as gonorrhea or chlamydia, your risk of getting those is higher than your risk for HIV.

My recommendation is that you go on and get tested for common STDs as soon as is convenient for you. They can be present without symptoms.  As for HIV, yor risk is rather low but testing is a good idea.  I would suggest that when you get back to the U.S.  you seek testing at 4 weeks following your exposure with one of the recently available combination HIV p24 antigen/HIV antibody (DUO) tests.  These will provide definitive results at that time.  We do not recommend PCR testing for HIV.  While the PCR is likely to become positive more quickly than other antibody detection tests (i.e. usual blood tests), at the present time the blood tests are becoming more and more sensitive and detecting infection earlier and earlier so that the time difference in detection between PCR test and antibody detection tests is becoming smaller and smaller and at present is, in general only a week or two.  In addition the time course over which the PCR tests become positive is less well described than for the blood tests and, as a result, it is difficult to make a definitive statement on what a negative PCR test means at any time within a few weeks of exposure to a HIV infected or possibly infected partner.  PCR tests are also more expensive than regular antibody tests.  Finally and most importantly, the false positive rate for PCR tests (i.e. a positive result in persons who do not have HIV) is higher than for blood tests.  Each of this on this Forum have seen a number of people who were worried needlessly because of false positive tests. For all of these reasons, we rarely recommend testing for HIV diagnosis using PCR.

Hello Doctor,

Just posting to update my situation. (I'm sure this will most likely be my last post in this thread before I finally put this matter to rest)

To get to the point, I did take a HIV PCR-DNA around 20 days post exposure. The result for that test was non-reactive. Today morning (8 weeks and 2 days after exposure) I took a HIV Combi Antigen/Antibody test (unknown brand) here in Colombia. I just picked up the result (only a few ours after being tested) and it says that my COI is 0.15 where anything less than 0.90 is non-reactive.

Is this result(s) definitive answer that I do not contracted HIV from the previous scenarios? Or should I still take a confirmatory exam at the 12/13 week mark?

The bad news is, however, that I was found HBV Antigen reactive (even though I had been vaccinated with 3 shots 10 years ago. This confirms what you said, meaning there are plenty other STD's beside HIV to be worried about, and which are easier to get infected from.

Finally, I'd like to thank you for your great help. It helped me get some shut-eye many a night.

Eiterh test would be fine but the PCR RNA test is the direct test for the virus.  EWH

Thank you Dr.

I have talked to the Department of Health and several communty organizations. No one seems to know what a HIV Combo/Duo test is... Thus, one last question before putting this matter to rest. I will visit a general doctor on saturday (two weeks post=exposure) where I am going to ask for a PCR test if the DUO is not available.

My final question is: What is the difference between the PCR-DNA and PCR-RNA tests? Which would be better for my situation (exactly 2 weeks)?

I would check with people at your local STD clinic (Department of Health) or a local HIV directed community based organization about the best way to etierh get a DUO test or a PCR to rule out infection.  Go with their advice.  EWH

Hello Doctor,

I apologize once more for bothering you (you must be tired from my continuous questioning, I know I am ), however, the test you recommended (Combo Ab/Ag test) is not as easily available here in South Florida as I would have hoped (most places I have called do not even seem to recognize it!). How should I proceed? I have been thinking in taking a separate p-24 antigen test before the 4 week mark, while at the same time also taking a regular antibody test and combining the results. How effective would this be?  Would this be as accurate as the combo test? How should I proceed?

Also, to update my situation, I have noticed that the back of my tongue is covered in a white substance, and my throat and right ear have been discomforting since around 4 days post exposure (it has been 1 week and 3 days since possible exposure). Also diarrhea has come back for a couple of days (since 2 days ago) but it seems to have subsided. I keep relating sneezes, discomfort and weakness (both arms, mostly right, legs), chest and abdominal discomfort and tingling, general weakness and mostly everything else in my body to my possible HIV exposure.

I am trying to gather some resources to visit a doctor, but without any insurance, the costs are simply too much, leaving this forum (and yourself) as my only possible advice…

The onset of your symptoms is too early to be a manifestation of ARS.

No change in my assessment, your risk for infection is quite low.

Otherwise no change in my assessment.  I recommend that you just try to put your concerns aside until you choose to test.   At that time I am confident your tests will be negative and you can move on form this.  EWH

Hello Doctor,

I apologice for posting once more before the necessary tests have been taken since yesterday night I have developed stiffness in the left back part of the neck and today (3 days after possible exposure) I have woken up at around 8am and when I went to the bathroom I had diarrhea. The anxiety came back immediately and I almost forced myself tp throw up while showering.

Also, I checked today with the two places I got the girls from. The man from the first place (the one I had unprotected sex with) told me that most of the girls that live in the city have the HIV test done, but some girls (like the one I had) come from nearby cities for the weekends and that he does not know if they had the test recently or not. He also told me that the particular girl I had unprotected sex had left the day after (sunday) and that she is most likey not coming back to work with him (she had mentioned that she did not like working with him because he overcharged most of his girls).

I also remember a detail which is that we did not have any lubrication so when I penetrated her my penis streched a good deal inside her, which is where I most likely where I got the small abrasion in my penis (which has since almost healed completely).

The second place (the nicer of the two) told me that the girls had their tests up to date but with out elaborating further. I didnt push the question too much since I had protected sex on that site.

Now, once again I apologize for the long update, but my anxiety is back even stronger than before, making me think that this greatly increases my chances of being infected with HIV.

Once more, thank you very much Dr. Hook for your help.

Everything is clear, however, one last question so I can rest my mind (for a while at least). I have read from Dr Gonzales-Garcia (also a member of this very helpful site) that it is possible to get a DUO test (like you mentioned before) 2 weeks after initial exposure with 90% reliability the test results will be accurate. Would you agree?

Please reread my comments on the PCR test.  That summarizes my feelings. we do not recommend them.  A negative PCR test would be reassruing but a postive will create chaos for you while you wait to get it sorted out.  I've seen this happen too many times to endorse this non-recommeded test (accept for the peole who sell them).  

Looking for symptoms is  waste of time.  The symptoms , when they occur are non-specific and over half of peole do not develop symptoms.  

You will have to research costs on your own.  Testing in Helath Departmetns tends to be most reliable and least expensive.  

Your risk for HIV remains low.  EWH

The events I described happend only last night, and I really cant picture myself during the holydays with that same thought on my head.

But, regarding the PCR test, basically a negative result after 1 or 2 weeks exp. could not be called reliable while a 4 week blood test "combo" will give me a definite answer? In other words, will a PCR that comes out negative give me a better chance of coming out negative in the HIV p24 antigen/HIV antibody (DUO) tests?

Also, if I am indeed infected (I pray to God, Im not, even with the low chance you mentioned), would the symptoms occur soon? Would they appear 1 by 1 or all in a row?

Last, I know this may vary from state to state (I'm from South Florida), but what is the rough difference in costs between exams? I am currently uninsured, and being a student, I find myself very low on money for all of this exams.

Once again, thank you for you answer, it has calmed me a bit, though the torn on the side is still there...

2) Not satisfied (and now thinking even more irrationally than before), I went to meet her again at a place she was going to be at later (a sex club), but instead I ended up with another girl. We had protected sex but after minutes of trying I still did not reach climax. She did reach orgasm, and her vagina was very wet the few times I inserted my finger in it. Also I kissed her vagina once or twice before she came.  She then gave me a hand job without a condom and lubricated my penis with her saliva (I know man! what the hell was I thinking!!!) but I still did not reach climax. (She is an unknown HIV status)
3) So after that, I went for a 3rd round at another place, where I got this other girl. We deep kissed and rubbed for a bit, then she gave me a few licks, in two or three different opportunities, to the top of the head (where the urine comes out) but I immediately told her to stop and put my condom on. Other than that, she rubbed her vagina against me before putting the condom (but there was no penetration), but after that it was really quick, protected sex (if you can even still call it that...)
Now, I know what I did was BEYOND stupid and I really NEVER plan on doing it again. I am INCREDIBLY worried about this, couldn’t even get any sleep last night, and almost started to cry today and I keep thinking about what will be my life with HIV. I am leaving to the states in early December, and I could really use some help and insight. What are my best options? Certainly, a test is a must but which one is better? The faster the result the better since I really can’t picture myself living with this anxiety for three months. I was reading about the HIV NAT test, that it could provide results after only 11 days of exposure. Are there any other tests similar to this one that I can take soon to have at least a bit of peace of mind? Any other recommendations or suggestions and insight on my case? What are my chances of beign HIV positive?
Please help!!! I really never thought that this would happen to me but it did and being my fault, and it’s the worst feeling ever.......
